摘　　要：目的探讨肺癌组织中1-磷酸神经鞘氨醇受体1 (S1PR1)的表达及临床意义。方法收集南通市肿瘤医院52例肺癌患者组织标本,采用实时荧光定量PCR(RT-PCR)法检测肺癌组织及癌旁组织中S1PR1的表达水平,通过Oncomine数据库对S1PR1表达结果进行佐证,并分析其与临床病理参数之间的关系;利用ROC曲线,评价其表达水平对肺癌的诊断价值;Kaplan-Meier Plotter数据库分析其与肺癌预后的关系。结果肺癌组织中S1PR1的表达水平明显低于癌旁组织(P<0.01),Oncomine数据库共检索出9项符合筛选条件的研究,S1PR1在所有研究中都呈低表达(P<0.01)。S1PR1的表达与肿瘤大小和淋巴结转移有关(P<0.05),而与性别、年龄、吸烟情况、病理类型及TNM分期无明显相关性(P>0.05)。根据S1PR1表达水平绘制ROC曲线,曲线下面积(AUC)为0.711(95%CI:0.610~0.813),临界值取1.127时,S1PR1诊断肺癌的灵敏度和特异性均为71.2%。Kaplan-Meier生存分析发现低表达S1PR1的患者5年总生存期显著低于高表达患者(P<0.01)。结论肺癌组织中S1PR1表达水平下调,且低表达S1PR1的患者预后较差,S1PR1有望成为肺癌诊断和预后的分子标志物。Objective To investigate the expression of sphingosine-1 phosphate receptor 1(S1PR1)in lung cancer tissues and its clinical significance.Methods 52 tissues of lung cancer patients were collected from Nantong Tumor Hospital.The expression of S1PR1 was compared between lung cancer tissues and adjacent tissues by real-time PCR(RT-PCR).The S1PR1 gene expression results were confirmed by data from Oncomine database,and its relationship with clinical pathological parameter was analyzed.ROC curve was drawn to evaluate the diagnosis value of S1PR1 expression in lung cancer.The prognosis of lung cancer patients was retrieved from an online survival analysis tool,named“Kaplan-Meier Plotter”database.Results The expressions of S1PR1 in lung cancer tissues were lower than those in adjacent tissues(P<0.01).To validate this result,a total of 9 previous studies were selected from the Oncomine database,and the expression levels of S1PR1 were all decreased(P<0.01).The expression of S1PR1 was also correlated to tumor size and lymphatic metastasis(P<0.05),but not correlated with gender,age,smoking history,pathological type or TNM staging(P>0.05).S1PR1 expression showed a value of diagnosis in lung cancer by results of ROC curve.AUC of ROC curve was 0.711(95%CI:0.610-0.813).When the critical value was 1.127,the sensitivity and specificity were both 71.2%.Kaplan-Meier survival analysis showed that the total five-year survival were shorter in patients with low expression of S1PR1 than those in patients with high expression of S1PR1(P<0.01).Conclusion S1PR1 is down-regulated in lung cancer tissues,and patients with low expression of S1PR1 have a poor prognosis.S1PR1 could be used as a molecular marker for the diagnosis and prognosis of lung cancer.
