调强放射治疗的首次摆位误差分析及应用

摘　　要：目的利用千伏级锥形束CT(CBCT)采集接受调强放射治疗的肿瘤患者的首次摆位误差数据进行分析,并将分析结果应用于临床。方法对接受调强放射治疗的432例患者按头颈部、胸腹部、盆腔部分为头颈组、胸腹组、盆腔组,首次摆位后行CBCT扫描,分别获取腹背、头脚、左右3个平移方向的误差及旋转误差,并应用SPSS Statistics 26软件进行数据分析。结果头颈组3个方向的误差及旋转误差分别为(1.23±0.94)mm、(1.68±1.17)mm、(1.15±0.89)mm、(0.63±0.72)°;胸腹组3个方向的误差及旋转误差分别为(1.54±1.28)mm、(2.47±1.73)mm、(1.45±1.54)mm、(0.40±0.50)°;盆腔组3个方向的误差及旋转误差分别为(1.98±1.70)mm、(1.86±1.40)mm、(1.95±1.59)mm、(0.38±0.44)°。结论利用CBCT在首次治疗时行图像引导摆位,可有效纠正摆位误差;分析各部位的摆位误差,能够很好地总结科室的系统误差和随机误差,确定适宜科室的各部位临床靶体积(CTV)外放边界,进一步提高靶区大小的精确性,减少放射治疗的不良影响。Objective To analysis the tumor patients'first set error data which collected by cone beam CT in intensity modulated radiation therapy(IMRT).Methods Acoording to the body regions,432 IMRT cases divided into three groups,there were head and neck group,chest and abdomen group,pelvic group.After the first set up,obtained direction errors(AP/SI/LR)and rotation errors from CBCT.Analysis using SPSS Statistics 26 software.Results Three directions and rotation errors in head and neck group were(1.23±0.94)mm,(1.68±1.17)mm,(1.15±0.89)mm,(0.63±0.72)°;Three directions and rotation errors in the chest and abdomen group were(1.54±1.28)mm,(2.47±1.73)mm,(1.45±1.54)mm,(0.40±0.50)°;Three directions and rotation errors in the pelvic group were(1.98±1.70)mm,(1.86±1.40)mm,(1.95±1.59),(0.38±0.44)°respectively.Conclusions Using the CBCT image guide the set up during the first treatment can effectively correct the set up errors,analyze the set up errors can sum up the systematic error and random error of the local section.Confirming the suitable margins for clincal target volume(CTV)in local section,which can improve the accuracy of target effectively and reduce the side effects of radiation therapy.
