摘　　要：目的探讨头颈肩膜颈段开口在头颈部肿瘤(HNC)放射治疗患者体位固定中的应用价值。方法选取2018年3月至2019年8月医院肿瘤放射治疗中心收治的60例HNC放射治疗患者作为研究对象,随机分为试验组和对照组,各30例。两组均采用碳素纤维底板、个体化发泡胶头颈肩枕和头颈肩膜固定,试验组头颈肩膜采取颈段开口处理,对照组头颈肩膜采取常规不开口处理,比较两组摆位误差及舒适度评分。结果两组在X、Y、Z轴线性方向及旋转方向平均摆位误差比较,差异均无统计学意义(P>0.05);试验组舒适度评分高于对照组,差异有统计学意义(P<0.05)。结论与常规不开口比较,头颈肩膜颈段开口在HNC放射治疗患者体位固定中的放射治疗精度不受影响,且在一定程度上提高了患者的舒适度。Objective To explore the application value of head-neck-shoulder film cervical opening in the position fixation of head neck tumor(HNC)patients undergoing radiotherapy.Methods From March 2018 to August 2019,60 cases of HNC radiotherapy patients admitted to the cancer radiotherapy center of the hospital were selected as the study objects and randomly divided into the experimental group and the control group,30 cases each.All patients were fixed with carbon fibre floor and individual styrofoam head-neck-shoulder pillow and head-neck-shoulder film.But for head and neck shoulder film,the experimental group was treated with cervical opening,while the control group was treated with cervical non opening.Comparison of two groups of set-up error data and comfort score data.Results There was no statistical difference in the average errors of X,Y,Z axis direction and rotation direction between the two methods(P>0.05);the comfort score of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with conventional radiotherapy,head-neck-shoulder film and cervical opening method does not affect the positioning accuracy of radiotherapy,but at the same time improves the comfort of patients in the treatment to a certain extent.
