中医药治疗慢性萎缩性胃炎及胃癌前病变的医案数据挖掘分析

摘　　要：目的采用数据挖掘方法分析慢性萎缩性胃炎(CAG)及胃癌前病变(PLGC)的相关医案,总结中医药治疗CAG及PLGC的经验。方法检索中国学术期刊全文数据库(CNKI)、维普中文期刊数据库(VIP)、万方数据库2009年1月1日-2019年1月18日近10年关于CAG及PLGC治疗经验方面的文献,筛选有效医案进行数据挖掘。结果最终纳入文献241篇,涉及专家学者168位,医案308则。症状以胃痛出现频次最多;病位证素主要是胃、脾、肝等6个;病性证素是气虚、血瘀、气滞、热、湿等12个;治法为补益法、理血法、理气法、清热法、祛湿法等9种;治疗使用的中药共260种,出现频次≥20%的中药有甘草、白术、半夏、茯苓、陈皮等共28种;甘草与茯苓是最常见的药物配伍;以经方加减的处方中,四君子汤、半夏泻心汤、香砂六君子汤、柴胡疏肝散出现频率较高。结论CAG及PLGC在症状、证候、治法、用药方面有一定规律可循,数据挖掘在医案研究中具有较好的应用价值。Objective Data mining method was used to analyze medical cases related to chronic atrophic gastritis(CAG)and gastric precancerous lesions(PLGC),and to summarize the experience in the treatment of CAG and PLGC with traditional Chinese medicine(TCM).Methods A comprehensive search of the CNKI,Weipu Chinese Journal Database(VIP),and Wanfang Database from January 1,2009 to January 18,2019 for nearly 10 years of literature on the treatment experience of CAG and PLGC,screening Effective medical records for data mining.Results 241 literatures were finally included,involving 168 experts and scholars,and 308 medical cases.The most frequent symptom was stomachache.The location elements are mainly stomach,spleen and liver,and the syndrome elements are 12 qi deficiency,blood stasis,qi stagnation,heat and dampness,etc..There are 9 kinds of blood methods,qi-regulating methods,heat-clearing methods and dampness removing methods.There were 260 kinds of herbs used in treatment,and there were 28 kinds of TCM with a frequency of≥20%,such as liquorice,atractylodes,pinellia ternata,tuckahoe,orange peel,etc.Licorice and poria cocos are the most common herb combinations.Among the prescriptions,Sijunzi Decoction,Banxia Xiexin Decoction,Xiangsha Liujunzi Decoction and Chaihu Shugan Powder appeared more frequently.Conclusion CAG and PLGC have certain rules to follow in terms of symptoms,syndromes,treatment and medication.Data mining has good application value in medical case study.
