摘　　要：目的:分析局部任意皮瓣修复头面部皮肤恶性肿瘤彻底切除后创面的效果。方法:回顾总结2017年7月-2019年7月笔者医院皮肤科收治的头面部皮肤恶性肿瘤切除术后行局部任意皮瓣修复手术80例患者临床资料,对患者修复总有效率进行统计,并比较修复前和修复半年后创面色泽、美观度、功能、情绪状态评分和复发率情况。结果:80例头面部皮肤恶性肿瘤患者,修复总有效率为95.00%(76/80)。修复后,患者创面色泽评分、美观度评分、功能评分较修复前均升高,患者90项症状自评(Symptom check list-90,SCL-90)心理因子评分、焦虑自评量表(Self-rating anxiety scale,SAS)和抑郁自评量表(Self-rating depression scale,SDS)评分较修复前均降低,差异均有统计学意义(P<0.05)。术后随访半年,复发率为2.50%(2/80)。结论:局部任意皮瓣用于头面部皮肤恶性肿瘤切除术后创面修复,能显著改善患者面部美观度和功能状态,且移植皮瓣色泽更好,效果持久性更好,术后复发少,患者心理状态佳。Objective To analyze the effect of local arbitrary skin flap on the wound surface of the head and face after the complete resection of large area of malignant skin tumors.Methods Review between July 2017 and July 2019 of first admitted in our hospital dermatological department initiated local skin flap to repair any facial skin malignant tumor resection surgery clinical data of 80 patients,for patients to repair for statistical total effective rate,and repair the wound after half a year before color,aesthetic,functional evaluation,emotional state and repair the recurrence rate after half a year.Results The total effective rate was 95.00%(76/80).After repair,the wound color score,aesthetic score and functional score of the patients were higher than those before repair,and the scores of SCL-90,SAS and SDS were decreased,the differences were statistically significant(P<0.05).The patients were followed up for half a year,the recurrence rate was 2.50%(2/80).Conclusion Local arbitrary skin flap used for wound repair after resection of malignant tumor of head and face can significantly improve the facial beauty and functional state of patients,and the transplanted skin flap has better color,better lasting effect,less postoperative recurrence and good psychological state of patients.
