间接法树脂高嵌体修复治疗磨牙残冠的疗效观察

摘　　要：目的:探讨间接法树脂高嵌体修复对磨牙残冠患者的疗效及美观性的影响。方法:选取笔者医院磨牙残冠需行修复治疗患者120例,根据树脂高嵌体修复方式分成两组,分别为间接法组61例(患牙67颗)和直接法组59例(患牙64颗)。两组均于术后半年入院复查,比较两组修复效果,记录手术时间、粘接面积和微剪切强度。分别在患者术前、术后6个月评估其修复美观度,同时记录术后6个月内的并发症发生率。结果:间接法组成功率为94.03%,高于直接法组的81.25%,两组比较差异有统计学意义(P<0.05);间接法组的手术时间长于直接法组,微剪切强度高于直接法组,差异有统计学意义(P<0.05);间接法组治疗后的牙龈着色、边缘密合性、排列评分与总分高于直接法组,差异有统计学意义(P<0.05)。间接法组与直接法组的根尖周病变、牙根折裂、牙齿松动及桩核折断发生率比较无统计学意义(P>0.05)。结论:与直接法树脂高嵌体修复相比,间接法的手术时间相对较长,但能提高修复成功率与微剪切强度,且美容效果更好,不增加并发症风险。Objective To explore the effect of indirect resin inlay restoration on the curative effect and aesthetics of molar residual crown patients.Methods 120 cases of molar residual crown need to be repaired and treated in our hospital were selected.According to the method of resin inlay restoration,the patients were divided into two groups:indirect group (n=61,affected teeth=67),direct group (n=59,affected teeth=64).The two groups were reexamined half a year after operation,and the repair effect was compared between the two groups.The operation time,adhesive area and micro-shear strength were recorded.The aesthetics of the patients were evaluated before and 6 months after operation,and the incidence of complications within 6 months after operation was recorded.Results The power of indirect method was 94.03%,higher than that of direct method group (81.25%,P<0.05).The operation time of indirect method group was longer than that of direct method group,and the micro-shear strength was higher than that of direct method group (P<0.05).The gingival staining,marginal cohesion,arrangement score and total score of indirect method group after treatment were higher than those of direct method group (P<0.05).There was no significant difference in the incidence of periapical lesions,root fracture,tooth loosening and post-core fracture between indirect method group and direct method group (P>0.05).Conclusion Compared with direct resin inlay repair,indirect method has a longer operation time,but it can improve the success rate and micro-shear strength,and has better cosmetic effect without increasing the risk of complications.
