摘　　要：目的分析BLR-rec(双眼外直肌后徙术)和R&R(单眼一退一截术)治疗儿童基本型间歇性外斜视的临床疗效。方法纳入郑州大学附属儿童医院2018年4月-2019年4月收治的90例基本型间歇性外斜视儿童,根据诊断结果分为实验A组(双眼交替注视)和实验B组(单眼注视),实验A组45例患者采纳BLR-rec手术治疗,实验B组45例患者采纳R&R手术治疗,对比两组手术成功率、外斜漂移量、视远斜视度、视近斜视度。结果实验A组手术成功率(86.67%)与实验B组(84.44%)比较无显著差异(P>0.05);实验A组术后1月外斜漂移量、视远斜视度、视近斜视度与实验B组比较无显著差异(P>0.05);实验A组术后2月、4月外斜漂移量、视远斜视度、视近斜视度均显著比实验B组低(P<0.05)。结论BLR-rec手术与R&R手术在治疗儿童基本型间歇性外斜视中近期疗效相当,但随着时间的推移,R&R手术患者外斜漂移量会逐渐增加,这是导致R&R手术远期疗效降低的一个重要原因。Objective To analyze the clinical effect of BLR-rec(bilateral lateral rectus recession)and R&R(unilateral recessresect)in the treatment of basic intermittent exotropia in children.Methods 90 children with basic intermittent exotropia admitted to the children’s hospital of Zhengzhou university from April 2018 to April 2019 were divided into experimental group A(alternate binocular fixation)and experimental group B(monocular fixation),45 patients in group A and 45 patients in group B were treated with BLR-rec,and 45 patients in group B were treated with R&R.The successful rate of operation,the amount of deviation,the degree of squint and the degree of squint were compared between the two groups.Results There was no significant difference between experimental group A(86.67%)and experimental group B(84.44%)(P>0.05);There was no significant difference between group A and group B(P>0.05)in the amount of deviation,the degree of hyperopia and the degree of nearsightedness 1 month after operation.The deviation of exotropia,diplopia and myopia in group A were significantly lower than those in group B(P<0.05).Conclusion BLR-rec surgery and R&R surgery have similar short-term efficacy in the treatment of basic intermittent exotropia in children,but with the passage of time,the amount of exotropia drift will increase gradually in R&R surgery,this is one of the important reasons for the decrease of the long-term effect of R&R surgery.
