加味黄芪建中汤联合四联疗法对T2DM合并PU患者的临床疗效和PG的影响

摘　　要：目的 探讨加味黄芪建中汤联合四联疗法治疗2型糖尿病(type 2 diabetes mellitus,T2DM)合并消化性溃疡(peptic ulcer,PU)患者临床疗效观察以及对血清胃蛋白酶原(PG)的影响.方法 选取2018年7月至2019年9月住院治疗的96例T2DM合并PU患者,并按照随机数字表随机将其分为中药观察组与西药对照组,西药对照组患者接受四联疗法治疗及常规降糖治疗;中药观察组患者在西药对照组基础上服用加味黄芪建中汤.于治疗前后观察两组患者的临床疗效、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)水平.②血清PG活力检测.以全自动免疫分析、 溃疡愈合质量、Hp根治情况、不良反应及血清PG活力表达水平.结果 中药观察组治疗总有效率为93.75%(45/48),西药对照组治疗总有效率为79.17%(38/48),中药观察组临床疗效明显高于西药对照组,差异具有统计学意义(P<0.05);且中药观察组患者FPG、2 hPG、HbA1c、溃疡愈合质量、Hp根治情况、血清PG水平均较西药对照组均有明显改善(P<0.05),且无明显不良反应.结论 加味黄芪建中汤联合四联疗法治疗T2DM合并PU患者的临床疗效显著,可明显控制T2DM合并PU患者的血糖情况,降低血清PGⅠ、PGⅡ水平.Objective To explore the clinical effect of modified Huangqi Jianzhong decoction combined with quadruple therapy in the treatment of type 2 diabetes mellitus with peptic ulcer and its effect on serum pepsinogen(PG).Methods 96 patients with type 2 diabetes mellitus complicated with peptic ulcer who were hospitalized from July 2018 to September 2019 were selected and randomly divided into Chinese medicine observation group and western medicine control group according to the random number table method.The patients in the western medicine control group received quadruple therapy and conventional hypoglycemic treatment.The patients in the Chinese medicine observation group took in modified Huangqi Jianzhong decoction on the basis of the western medicine control group.Before and after treatment,the clinical effect,fasting blood glucose,2-h postmeal blood glucose,glycosylated hemoglobin,ulcer healing quality,radical cure of Hp,adverse reactions and serum PG activity expression level of the two groups were observed.Results The effective rate of the Chinese medicine observation group was 93.75%(45/48),and that of the Western medicine control group was 79.17%(38/48).The clinical effect of the Chinese medicine observation group was significantly higher than that of the western medicine control group,and the difference was statistically significant(P<0.05).In addition,fasting blood glucose,2-h postmeal blood glucose,glycosylated hemoglobin,ulcer healing quality,radical cure of Hp,and serum PG level in the Chinese medicine observation group were significantly improved compared with the western medicine control group(P<0.05),and there were no significant adverse reactions.Conclusion The clinical effect of modified Huangqi Jianzhong decoction combined with quadruple therapy on type 2 diabetes patients with peptic ulcer is remarkable,which can obviously control the blood glucose in patients with type 2 diabetes mellitus and peptic ulcer,lower levels of serum PGⅠand PGⅡ.
