岩舒注射液配伍白蛋白紫杉醇治疗恶性腹腔积液疗效及安全性分析

摘　　要：目的探究白蛋白紫杉醇联合岩舒注射液治疗恶性腹腔积液的疗效性及安全性。方法选取本科48例卵巢恶性肿瘤伴有腹腔积液患者,采用随机数字表法将所有患者平均分成3组,每组16例,向A组采用白蛋白结合型紫杉醇260mg/m^2静滴,腹腔灌注岩舒注射液30 mL(3次/周);向B组单纯采用白蛋白结合型紫杉醇260 mg/m^2静滴;向C组单纯灌注复方苦参注射液30 mL(3次/周);每周期入院时行超声探查腹腔积液量的变化及临床症状和生活质量的改变。结果(1)A组有效率73.33%,临床获益率93.75%;B组有效率56.25%,临床获益率81.25%;C组有效率37.50%,临床获益率68.75%。A与B及A与C组间比较,有效率和临床获益率比较差异均有统计学意义(P<0.05)。(2)A组生活质量改善率为86.66%;B组改善率为60.00%,C组改善率为40.00%;A与B及A与C组间比较改善率比较差异具有统计学意义(P<0.05)。(3)A组不良反应发生率明显低于B组,差异具有统计学意义(P<0.05)。结论白蛋白结合型紫杉醇配伍岩舒注射液可有效抑制腹腔积液的形成,降低化疗不良反应,改善患者生活质量,效果较好,且安全性较高。Objective To investigate the clinical efficacyand safety of Compound Matrine Injection combined with Abraxane in the treatment of malignant ascites.Methods Patients(48 cases)with medium-term and advanced ovarian cancer with malignant ascites in Department of Gynaecology of Hubei Cancer Hospital were enrolled in this study.According to the different treatment plans,patients were divided into three groups(n=16).The patients in the A group were administered with Abraxane Injection by intravenous drip(260 mg/m^2)and administered with Compound Matrine Injection(three times a week).The patients in the B group were administered with Abraxane Injection by intravenous drip(260 mg/m^2)only.The patients in the C group were administered with Compound Matrine Injection only(three times a week).Three groups were all treated for three courses.The efficacy and improvement of life quality in two groups were evaluated and theadverse reaction were also compared.Results(1)After treatment,the total effective rate of A group was 73.33%,and the clinical benefit rate was 93.75%,while the total effective rate of B group was 56.25%,and the clinical benefit rate was 81.25%.The total effective rate of C group was 37.50%,and the clinical benefit rate was 68.75%and there were differences between A and B or between A and C(P<0.05).(2)The improvement rates of life quality in the 3 groups were 86.66%,60.00%and 40.00%,respectively and there were differences between A and B or between A and C(P<0.05).(3)After treatment,the cases of bone marrow suppression,gastrointestinal reaction in the A group were significantly lower than those of B group and there were differences between two groups(P<0.05).Conclusion Compound Matrine Injection combined with Abraxane not only can increase the quality of life,but also can inhibition of malignant ascites with few adverse reactions,which has a certain clinical value.
