玻璃体腔注射曲安奈德联合577nm微脉冲激光治疗糖尿病性黄斑水肿患者的临床疗效

摘　　要：目的研究玻璃体腔注射曲安奈德联合577 nm微脉冲激光治疗糖尿病性黄斑水肿(DME)患者的疗效。方法选取我院80例(80眼)DME患者(2017年3月~2019年1月),随机数字表法分为对照组(n=40)和研究组(n=40)。对照组采用577 nm微脉冲激光治疗,研究组577 nm微脉冲激光治疗后玻璃体腔注射曲安奈德。对比两组治疗效果、治疗前后黄斑中心凹厚度和最佳矫正视力的差值。结果研究组治疗总有效率[95.00%(38/40)]高于对照组[77.50%(31/40)](χ2=5.165,P=0.023);治疗前、治疗1个月后,研究组黄斑中心凹厚度差值、最佳矫正视力的差值均高于对照组(t1=9.989,P1=0.000;t2=14.628,P2=0.000)。结论采用玻璃体腔注射曲安奈德联合577 nm微脉冲激光治疗DME,能进一步降低患者黄斑中心凹厚度,改善视力水平,显著提高治疗效果。Objective:To study the effect of intravitreal triamcinolone acetonide injection combined with 577 nm micro-pulse laser in the treatment of diabetic macular edema(DME).Methods Eighty(80 eyes) patients with DME in our hospital(from March 2017 to January 2019) were selected and divided into control group(n=40) and study group(n=40) according to the random number table method.The control group was treated with 577 nm micropulse laser,the study group was treated with vitreous cavity injection of triamcinolone combined with 577 nm micropulse laser treatment,and after laser treatment,triamcinolone was injected in the vitreous cavity.The difference between the two groups’ treatment effects,the thickness of the macular fovea,and the best corrected visual acuity before and after treatment were compared.Results The total effective rate of treatment in the study group [95.00%(38/40)] was higher than that in the control group [77.50%(31/40)](χ2=5.165,P=0.023);before treatment and after 1 month of treatment,the difference in macular foveal thickness and the best corrected visual acuity in the study group were higher than those in the control group(t1=9.989,P1=0.000;t2=14.628,P2=0.000).Conclusion Vitreous injection of triamcinolone acetonide combined with 577 nm micro-pulse laser in the treatment of DME can further reduce the thickness of macular fovea,improve visual acuity and significantly improve the therapeutic effect.
