摘  要:目的:应用静息态fMRI对伴嗅觉功能障碍的早期帕金森患者进行局部一致性(ReHo)分析,帮助早期诊断该病。方法:选取30例早期帕金森病伴嗅觉障碍患者(患者组)及30例匹配的健康人(对照组),采用GE Signa HDX 1.5 T超导MRI扫描仪行静息态f MRI。分析2组的ReHo图,并行双样本t检验(P<0.01,Alpha Sim校正),判断脑功能激活区。结果:患者组较对照组差异有统计意义的激活脑区为右侧小脑半球、右侧海马旁皮质(BA36)、右侧颞下回(BA20)、双侧梭状回(BA37)、右侧颞上回(BA22)、右侧体感联合皮质(BA7)。患者组较对照组差异有统计意义的抑制脑区为双侧豆状核、左侧前额叶(BA10)、右侧前扣带皮质(BA32)。结论:静息态下帕金森病伴嗅觉功能障碍患者与健康人的脑功能区ReHo值存在差异,且与其更复杂的临床症状有关,静息态fMRI有助于早期诊断评估帕金森病。Objective:Using ReHo value in resting-state fMRI(rs-fMRI) to analyze the variations on Parkinson’s patients with olfactory dysfunction.Methods:30 patients with early Parkinson’s disease and olfactory dysfunction and 30 matched healthy people were recruited in this study,all subjects underwent fMRI with GE Signa HDX 1.5 T superconducting MRI.Two sample t-test to analyze ReHo to find out the differences of the two groups.Results:Compared with the control group,the positive activated areas in the Parkinson’s disease group were right cerebellum,right parahippocampa gyrus(BA36),right inferior temporal gyrus(BA20),bilateral fusiform lobe(BA37),right superior temporal gyrus(BA22),right somatosensory association cortex(BA7).Compared with the control group,the negative activated areas in the Parkinson’s disease group were lentiform nucleus,left frontopolar area(BA10),right dorsal anterior cingulate cortex(BA32).Conclusions:ReHo changes in the brain region of early Parkinson’s patients with olfactory dysfunction in rs-fMRI are different from that of normal people,which is related to many complicated clinical symptoms.
