双相情感障碍丙戊酸钠治疗过程中POLG基因突变1例分析

摘　　要：丙戊酸钠作为一种常用的心境稳定剂,在双相情感障碍患者中广泛应用,对于双相情感障碍快速循环型和混合发作型也有效。丙戊酸钠对肝功能的损害不可忽视,具有POLG基因突变的患者,在使用丙戊酸钠时会加重肝功能的损害,甚至出现肝衰竭,导致患者死亡。本文报道1例在早期使用丙戊酸钠治疗双相情感障碍过程中检测到POLG基因突变并及时停用丙戊酸钠的病例。并通过此病例,建议精神科医师在使用丙戊酸钠治疗前,进行POLG基因分析,尤其是怀疑有线粒体疾病的患者。Sodium Valproate as a mood stabilizer is widely used in patients with bipolar disorder,which is also effective for rapid cycling and mixed episodes about bipolar disorder.The damage of Sodium Valproate to liver function cannot be ignored.The use of Sodium Valproate in patients with POLG gene mutation will aggravate the damage of liver function,and even cause liver failure,leading to death of the patients.This article reports a case in which POLG gene mutation was detected during the early use of Sodium Valproate in the treatment of bipolar disorder and Sodium Valproate was discontinued in time.And through this case,it is recommended that psychiatrists should perform POLG gene analysis before treatment with Sodium Valproate,especially in patients suspected with mitochondrial disease.
