养胃颗粒治疗慢性萎缩性胃炎脾虚气滞证临床研究

摘　　要：目的:观察养胃颗粒治疗慢性萎缩性胃炎脾虚气滞证的临床疗效。方法:选取慢性萎缩性胃炎患者150例,按随机数字表法分为对照组和观察组各75例。2组均按四联疗法治疗,对照组服用多潘立酮片,观察组在对照组治疗基础上服用养胃颗粒。比较2组脾虚气滞证症状评分、临床疗效、免疫功能、血液流变学指标。结果:治疗后,2组脾虚气滞证各项症状评分较治疗前降低(P<0.05);观察组脾虚气滞证各项症状评分低于对照组(P<0.05)。观察组总有效率为92.00%,高于对照组80.00%,差异有统计学意义(P<0.05)。治疗后,观察组的CD3+、CD4+水平显著上调且明显高于对照组,CD8+水平显著下调且明显低于对照组(P<0.05);对照组治疗前后CD3+、CD4+、CD8+变化不明显(P>0.05)。治疗后,2组血液流变学指标血浆黏度、全血黏度、纤维蛋白原水平较治疗前下降(P<0.05);且观察组各项血液流变学指标水平明显低于对照组(P<0.05)。结论:养胃颗粒治疗慢性萎缩性胃炎脾虚气滞证疗效确切,可改善患者的免疫功能和血液流变学指标。Objective:To observe the clinical effect of Yangwei granules for chronic atrophic gastritis with spleen deficiency and qi stagnation syndrome.Methods:A total of 150 cases of patients with chronic atrophic gastritis were selected and divided into the control group and the observation group according to random number table method,75 cases in each group.Both groups were given quadruple therapy;the control group was given the oral administration of domperidone tablets;the observation group was additionally given the oral administration of Yangwei granules based on the treatment of the control group.The symptom scores of spleen deficiency and qi stagnation syndrome,the clinical effect,immune function and hemorheology indexes in the two groups were compared.Results:After treatment,each symptom score of spleen deficiency and qi stagnation syndrome in both groups was lower than that before treatment(P<0.05);each symptom score of spleen deficiency and qi stagnation syndrome in the observation group was lower than that in the control group(P<0.05).The total effective rate was 92.00%in the observation group,higher than that of 80.00%in the control group,the difference being significant(P<0.05).After treatment,the levels of CD3+and CD4+in the observation group were significantly up-regulated,and higher than those in the control group;the levels of CD8+were significantly down-regulated,and lower than those in the control group(P<0.05).The levels of CD3+,CD4+and CD8+in the control group had no significant change before and after treatment(P>0.05).After treatment,hemorheological indexes including plasma viscosity,whole blood viscosity and the levels of fibrinogen in both groups were decreased when compared with those before treatment(P<0.05);hemorheological indexes in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:The application of Yangwei granules has significant curative effect in treating chronic atrophic gastritis with spleen deficiency and qi stagnation syndrome,which can
