摘  要:目的探讨不同类型胃食管反流病(GERD)患者食管组织蛋白酶激活受体-2(PAR-2)和环氧合酶-2(COX-2)表达及与炎性因子的相关性。方法选取2016年8月—2017年12月张家口市第一医院收治的GERD患者98例,根据GERD不同分型将患者分为Barrett食管(BE)组33例,糜烂性食管炎(EE)组31例,非糜烂性反流病(NERD)组34例,另外选取同期在本院进行电子胃镜检查的正常者30例作为正常组。苏木精-伊红(HE)染色法观察各组食管组织病理学形态;RT-PCR法检测各组食管组织中PAR-2和COX-2 mRNA表达情况;ELISA法检测各组血清炎性因子水平;Pearson相关性分析GERD患者食管组织中PAR-2 mRNA和COX-2 mRNA相对表达水平与血清炎性因子水平的关系。结果正常组食管组织病理学形态正常,无炎性细胞浸润;BE组可见化生的柱状上皮,EE组食管组织中出现鳞状上皮增生、大量炎性细胞浸润及黏膜糜烂等,NERD组只有少量鳞状上皮增生及炎性细胞浸润。与正常组比较,BE组、EE组和NERD组食管组织中PAR-2 mRNA和COX-2 mRNA相对表达水平均明显升高(P均<0.05);EE组和NERD组食管组织中PAR-2 mRNA和COX-2 mRNA相对表达水平均明显低于BE组(P均<0.05),NERD组明显低于EE组(P均<0.05)。与正常组比较,BE组、EE组和NERD组血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-1β(IL-1β)水平均明显升高(P均<0.01);EE组和NERD组血清TNF-α、IL-6、IL-8、IL-1β水平均明显低于BE组(P均<0.05),NERD组明显低于EE组(P均<0.05)。GERD患者食管组织中PAR-2 mRNA和COX-2 mRNA相对表达水平与血清炎性因子水平均呈明显正相关(P均<0.05)。结论GERD患者食管组织中PAR-2和COX-2 mRNA高表达,存在炎症反应,且以Barrett食管炎患者PAR-2和COX-2 mRNA表达水平和炎性程度最高,同时GERD患者食管组织中PAR-2和COX-2 mRNA表达水平与血清炎性因子水平呈正相关性。Objective It is to investigate the expression of protease activated receptor-2(PAR-2)and cyclooxygenase-2(COX-2)in patients with different types of gastroesophageal reflux disease(GERD)and their correlation with inflammatory factors.Methods A total of 98 GERD patients admitted to the First Hospital of Zhangjiakou from August 2016 to December 2017 were selected and were divided into 3 groups according to their GERD classifications:Barrett esophagus(BE)group(33 cases),erosive esophagitis(EE)group(31 cases)and non-erosive reflux disease(NERD)group(34 cases),and 30 normal cases who underwent electronic gastroscopy in our hospital during the same period were selected as the normal group.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of esophageal tissues in each group;reverse transcription-polymerase chain reaction(RT-PCR)was used to detect the expression of PAR-2 and COX-2 in esophageal tissues of each group;the levels of serum inflammatory factor in each group were determined;Pearson correlation analysis was used to observe the relationship between the relative expression levels of PAR-2 mRNA and COX-2 mRNA in esophageal tissue specimens of GERD patients and levels of serum inflammatory factor.Results The esophageal histopathology of the normal group was normal without inflammatory cell infiltration;metaplastic columnar epithelium was found in the BE group,squamous epithelial hyperplasia appeared in the esophageal tissue with a large number of inflammatory cell infiltration and mucosal erosion in the EE group,a little of squamous epithelial hyperplasia and inflammatory cell infiltration was found in NERD group.Compared with the normal group,the relative expression levels of PAR-2 mRNA and COX-2 mRNA in esophageal tissues of BE group,EE group and NERD group were all increased(P<0.05);the relative expression of PAR-2 mRNAand COX mRNA in esophageal tissues of EE group and NERD group were lower than that of the BE group(P<0.05),and the NERD group was lower than the EE group(P<0.05).Compared w
