摘 要:目的研究肺癌患者中的血浆热休克蛋白90α(Hsp90α)水平,探讨其在肺癌患者中的临床应用价值。方法选取2018年5月至2019年8月住院确诊的118例肺癌患者(其中肺腺癌50例,鳞癌47例,小细胞肺癌21例)和肺炎患者及健康体检者各30例作为检测对象,对其进行Hsp90α、CA72-4、CEA、CYFRA 21-1、NSE、ProGRP和SCC等肿瘤指标的检测并对检测结果进行统计分析。结果肺癌患者血浆Hsp90α蛋白水平明显高于正常对照组(P<0.0001),按其组织学分类其各类与正常组相比亦具有统计学意义(P<0.001)。通过绘制ROC曲线判断其诊断效能,分析血浆Hsp90α在肺癌中的诊断价值,使用75.08 ng/mL的最佳诊断临界值其曲线下面积(AUC)为0.802(95%CI 0.736~0.858),敏感性和特异性分别为72.03%和86.67%。联合检测其他的肺肿瘤指标对肺癌进行预测其AUC为0.966(95%CI 0.928~0.987),敏感性和特异性分别达到88.98%和96.67。结论研究证明血浆Hsp90α蛋白可作为肺癌的一种辅助诊断方法。通过联合检测其他的肿瘤标志物指标来预测肺癌的诊断效能,明显高于单项指标,进一步提高了肺癌的检出率。Objective To study the value of heat shock protein 90 α(Hsp90 α) in patients with lung cancer, and to explore the clinical value of Hsp90α in patients with lung cancer. Methods Statistical analysis of 118 patients with lung cancer(including 50 cases of lung adenocarcinoma, 47 cases of squamous cell carcinoma, 21 cases of small cell lung cancer),who were definitely diagnosed in hospital from May 2018 to August 2019 and 30 patients with pneumonia and 30 healthy people were taken as normal control group analyzed including Hsp90α, CA72-4, CEA, CYFRA21-1, NSE, ProGRP and SCC. Results The level of plasma Hsp90α protein in patients with lung cancer was significantly higher than that in the normal control group(P<0.0001). According to its histological classification, its types were also statistically significant compared with the control group(P<0.001). Evaluation of the diagnostic efficacy of ROC curve and the diagnostic value of plasma Hsp90 α in patients with lung cancer. Using the best diagnostic threshold of 75.08 ng/mL, the area under the curve(AUC) is 0.802(95%CI 0.736~0.858), the sensitivity and specificity were 72.03% and 86.67%, respectively. The combined diagnostic efficacy of Hsp90 α and other lung cancer indexes was predicted. The area under the curve AUC was 0.966(95%CI 0.928~0.987), the sensitivity and specificity were 88.98% and 96.67, respectively. Conclusion It has been proved that plasma Hsp90 α protein can be used as an assistant diagnostic method for lung cancer. The diagnostic efficiency of lung cancer combined with other tumor indicators is significantly higher than the detection of single indicators, which can further improve the detection rate of lung cancer.
