预后营养指数与肝癌患者住院天数的相关性

摘　　要：目的探讨围手术期预后营养指数(PNI)与肝癌患者住院天数的相关性。方法收集了惠州市第一人民医院肝胆小儿外科在2018年1月至2019年6月收治的首次接受肝癌切除术的52例原发性肝癌患者资料。其中男42例,女10例,中位年龄59岁。通过本单位医学伦理会的审查。根据患者术前及术后第1、3、7天的检查,计算PNI值,并分成分为PNI-0、PNI-1、PNI-3、PNI-7四组。采用one-way ANOVA分析不同时间点PNI值的差异。Pearson相关分析不同时间点PNI值与住院天数的关系。用受试者工作特征(ROC)曲线确定最佳界值。结果单因素方差分析显示PNI-0组(48.14±6.05)、PNI-1组(30.39±3.02)、PNI-3组(35.27±3.40)、PNI-7组(39.95±3.39)差异有统计学意义(F=170.021,P=0.000),且术后三组的PNI值均低于术前组。Pearson相关分析显示PNI-0组与住院天数呈强负相关(r=-0.6,P=0.000)。ROC曲线确定术前PNI值是51.1450时为最佳截点值。结论术前PNI值≤51.1450时,住院天数延长。Objective To investigate the correlation between perioperative prognostic nutritional index(PNI)and length of stay in hepatocellular carcinoma(HCC)patients.Methods The clinical data of 52 patients with HCC undergoing resection surgery in the Hui Zhou First People’s Hospital between January 2018 and June 2019 were analyzed.Among the patients,42 were males and 10 were females and the median age of 59 years old.The local ethical committee approval had been received.PNIs value were calculated according to the examination results at preoperation and the first、third、seventh day after surgery,then accordingly formed four groups(PNI-0,PNI-1,PNI-3,PNI-7).The PNIs differences were analyzed by using one-way ANOVA,and used the Pearson correlation analyzed the correlation between perioperative prognostic nutritional index and length of stay.Using ROC curve check the value of preoperative PNI.Results One-way analysis of variance showed statistically significant differences in PNI-0 group(48.14±6.05),PNI-1 group(30.39±3.02),PNI-3 group(35.27±3.40),PNI-7 group(39.95±3.39)(F=170.021,P=0.000),and the PNI values of the three groups after operation were lower than those of the preoperative group.Pearson correlation analysis showed that the PNI-0 group had a strong negative correlation with the length of hospital stay(r=-0.6,P=0.000).The ROC curve determined that the preoperative PNI value was 51.1450 as the best cut-off value.Conclusion Less than a preoperative PNI of 51.1450,the longer of hospitalization days.
