摘　　要：目的分析结肠癌并肠梗阻行腹腔镜手术治疗的效果。方法选取郑州中心医院2015年8月至2018年2月收治的40例结肠癌并肠梗阻患者为研究对象,根据手术方式将患者分为两组,每组20例。对照组行开腹手术治疗,观察组行腹腔镜手术治疗。比较两组围术期指标、炎症指标变化情况。结果观察组除手术时间外的各项围术期指标均明显优于对照组(P<0.05)。两组术前各项炎症指标比较,差异未见统计学意义(P>0.05)。与术前相比,两组术后72 h血清白细胞介素-10(IL-10)、C反应蛋白(CRP)以及肿瘤坏死因子-ɑ水平(TNF-ɑ)均明显升高(P<0.05);与对照组相比,观察组术后72 h的IL-10、CRP以及TNF-ɑ均更低(P<0.05)。结论腹腔镜手术能够明显减轻结肠癌肠梗阻患者的机体应激反应程度,加快患者术后康复速度。Objective To analyze the effect of laparoscopic operation on colon cancer with ileus.Methods Forty patients with colon cancer and ileus admitted to Zhengzhou Central Hospital from August 2015 to February 2018 were selected as the study objects.The patients were divided into two groups according to the operation mode,with 20 cases in each group.The control group was treated by laparotomy and the observation group by laparoscopy.The changes of perioperative indexes and inflammatory indexes were compared between the two groups.Results In addition to the operation time,all the perioperative indexes in the observation group were significantly better than those in the control group(P<0.05).There was no significant difference between the two groups in preoperative inflammatory indexes(P>0.05).Compared with that before operation,the levels of IL-10,CRP and TNF-αin the two groups increased significantly at 72 h after operation(P<0.05).Compared with the control group,the levels of IL-10,CRP and TNF-αin the observation group were lower at 72 hours after operation(P<0.05).Conclusions Laparoscopic surgery can significantly reduce the degree of stress response of colon cancer patients with intestinal obstruction,and speed up the recovery of patients.
