血清AFU、AFP、GGT、LAP及APT联合检测在原发性肝癌中的诊断价值

摘　　要：目的探讨α-L-岩藻糖苷酶(AFU)、甲胎蛋白(AFP)、γ-谷氨酰转肽酶(GGT)、亮氨酸氨基肽酶(LAP)、异常凝血酶原(APT)五项联合在原发性肝癌中的临床诊断价值。方法选取2018年6月至2019年6月本院送检本中心实验室的180例患者标本,依据临床诊断分为肝癌组、肝脏良性疾病组和对照组,每组60例,回顾性分析3组患者血清检测数据。结果肝癌组AFU、AFP、GGT、LAP、APT血清水平与肝脏良性疾病组及对照组比较差异均有统计学意义(P<0.05),肝脏良性疾病组除APT水平外,AFU、AFP、GGT、LAP水平与对照组组间差异具有统计学意义(P<0.05);肝癌组和肝脏良性疾病组各指标单个检测阳性率与对照组比较比较差异均具有统计学意义(P<0.05),肝癌组与肝脏良性疾病组检测指标阳性率中,AFU、LAP和APT比较差异具有统计学意义(P<0.05),联合检测时差异无统计学意义,五项联合检测时灵敏度最高达84.12%,准确性为89.05%。结论血清AFU、AFP、GGT、LAP、APT五项联合检测能够提高原发性肝癌和良性肝病的准确效率,具有重要的临床价值,值得推广应用。Objective To explore the clinical diagnostic value ofα-L-fucosidase(AFU),alpha fetoprotein(AFP),γ-glutamyltranspeptidase(GGT),leucine aminopeptidase(LAP)and abnormal prothrombin(APT)in primary liver cancer.Methods From June 2018 to June 2019,180 samples of patients sent to central laboratory in our hospital were divided into liver cancer group,benign liver disease group and control group,with 60 cases in each group.According to the clinical diagnosis,and the serum test values of the three groups were analyzed retrospectively.Results The serum levels of AFU,AFP,GGT,LAP and APT in the three groups were significantly different from those in the benign liver disease group and the control group(P<0.05).The serum levels of AFU,AFP,GGT and LAP in the benign liver disease group were significantly different from those in the control group(P<0.05).There was significant difference between the positive rate of single detection and the control group,and there was statistical significance between the groups(P<0.05).Among the positive rate of detection indicators between the liver cancer group and the liver benign disease group,there was significant difference among the AFU,LAP and APT(P<0.05).The difference of joint detection time was small,and there was no significant difference.The sensitivity of the five joint detection was 84.12%,the accuracy increased to 89.05%.Conclusion The combined detection of serum AFU,AFP,GGT,LAP and APT can improve the accuracy of primary liver cancer and benign liver disease,which has important clinical value and is worth popularizing.
