摘 要:目的探讨外周血T淋巴细胞亚群在非小细胞肺癌患者诊断及治疗中的临床意义。方法选取2018年8月至2019年7月本院收治的160例非小细胞肺癌患者外周静脉血,其中(Ⅰ~Ⅱ期80例、Ⅲ~Ⅳ期80例),测定两组患者外周血T淋巴细胞亚群的绝对计数,比较组间差异;其中55例患者于本院行完整标准方案化疗,于第4个周期化疗结束4周后采集血样,比较疾病控制组患者(46例)及疾病进展组患者(9例)化疗前后T淋巴细胞亚群水平差异。结果Ⅲ~Ⅳ期患者CD4^+、CD3^+T细胞计数、CD4^+/CD8^+明显低于Ⅰ~Ⅱ期患者,差异有统计学意义(P<0.05),Ⅰ~Ⅱ期与Ⅲ~Ⅳ期组间CD8^+T细胞计数比较差异无统计学意义;疾病控制组患者化疗后CD4^+、CD8^+、CD3^+T淋巴细胞计数均明显高于化疗前,差异有统计学意义(P<0.05),化疗前后CD4^+/CD8^+差异无统计学意义;疾病进展组患者化疗前后的CD4^+、CD8^+、CD3^+T淋巴细胞计数及CD4^+/CD8^+比较差异无统计学意义。结论非小细胞肺癌患者随着肿瘤分期的增高,免疫抑制更明显,外周血T淋巴细胞亚群对非小细胞肺癌患者的免疫监测、肿瘤分期诊断、治疗效果评估具有指导意义。Objective To explore the clinical significance of peripheral blood T-lymphocyte subsets in diagnosis and treatment of patients with non-small cell lung cancer.Methods Blood samples were obtained from 160 patients with non-small cell lung cancer(including 80 patients in stageⅠ-Ⅱ,and 80 patients in stageⅢ-Ⅳ),the T-lymphocyte subsets were detected.55 patients received complete standard chemotherapy in our hospital,and blood samples were collected 4 weeks after the end of the fourth cycle of chemotherapy.The T-lymphocyte subsets of prechemotherapy and postchemotherapy were compared in the disease control group and disease progression group.Results Peripheral blood CD4^+,CD3^+,CD4^+/CD8^+in stageⅢ-Ⅳwere significantly lower than that in stageⅠ-Ⅱ(P<0.05),and there was no statistical difference in CD8^+between stageⅢ-ⅣandⅠ-Ⅱ.In the disease control group,the CD4^+,CD8^+,CD3^+of postchemotherapy were significantly higher than that of prechemotherapy(P<0.05),and there was no statistical difference in CD4^+/CD8^+between prechemotherapy and postchemotherapy.In the disease progression group,there was no statistically significant difference in CD4^+,CD8^+,CD3^+and CD4^+/CD8^+between prechemotherapy and postchemotherapy.Conclusion With the progression of non-small cell lung cancer,the immunological function is further suppressed.T-lymphocyte subsets counts have guide significance for immune surveillance,diagnosis and evaluation the therapeutic effect of the patients with non-small cell lung cancer.
