柴胡蚤休汤联合化疗治疗原发性肝癌临床疗效及对相关血清指标的影响

摘　　要：目的观察柴胡蚤休汤联合化疗治疗原发性肝癌的临床疗效及对相关血清指标的影响研究。方法将80例原发性肝癌患者随机分为对照组和观察组两组,各40例。其中对照组患者给予FAP化疗方案,观察组在对照组治疗方案的基础上给予柴胡蚤休汤。两组均连续治疗5 d,28 d重复1次,每1个周期为1个疗程。两组疗程均为2个疗程。2个疗程后比较两组的临床疗效,观察两组患者治疗前后Kamofsky评分变化和相关血清指标基质金属蛋白酶-2(matrix Metalloproteinases-2,MMP-2)、基质金属蛋白酶-9(matrix Metalloproteinases-9,MMP-9)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、胰岛素样生长因子-2(Insulin-like growth factor-2,IGF-2)、白介素-2(Interleukin-2,IL-2)的变化。结果治疗后,观察组和对照组的客观有效率分别为35.0%和22.5%,两组之间具有统计学差异(P<0.05)。治疗后,两组的Kamofsky评分均显著提高,且观察组的Kamofsky评分显著高于对照组(P<0.05)。经过治疗后,对照组血清中MMP-2、MMP-9、VEGF、IGF-2水平显著高于观察组,而血清中IL-2水平显著低于观察组(P<0.05)。治疗后对照组和观察组的化疗不良反应比较,观察组在呕吐、腹泻、乏力等不良反应方面均显著低于对照组(P<0.05)。结论采用柴胡蚤休汤联合化疗治疗原发性肝癌可以提高临床疗效,改善患者的生活质量和血清相关指标MMP-2、MMP-9、VEGF、IGF-2和IL-2的水平,减少了不良反应的发生,值得临床推广使用。Objective To observe the clinical efficacy of Chaihu Zaoxiu Decoction combined with chemotherapy in the treatment of primary hepatocellular carcinoma(PHC)and its influence on serum indexes.Methods Eighty patients with primary liver cancer were randomly divided into control group and observation group,40 cases in each group.The patients in the control group were given FAP chemotherapy regimen,while those in the observation group were given Chaihu Zaoxiu Decoction on the basis of the treatment regimen in the control group.Both groups were continuously treated for 5 days,repeated once every 28 days,and each cycle was a course of treatment.Both groups had two courses of treatment.The clinical efficacy of the two groups was compared after two courses of treatment.The changes of Kamofsky score and related serum indicators,matrix Metalloproteinases-2(MMP-2),matrix Metalloproteinases-9(MMP-9)and vascular endothelial growth factor were observed before and after treatment.Changes of H factor,VEGF,Insulin-like growth factor-2(IGF-2)and interleukin-2(IL-2)were observed.Results After treatment,the objective effective rates of the observation group and the control group were 35.0%and 22.5%,respectively.There was a statistical difference between the two groups(P<0.05).After treatment,the Kamofsky scores of the two groups were significantly improved,and the Kamofsky scores of the observation group were significantly higher than those of the control group(P<0.05).After treatment,the serum levels of MMP-2,MMP-9,VEGF and IGF-2 in the observation group were significantly lower than those in the control group,while the serum levels of IL-2 in the observation group were significantly higher than those in the control group(P<0.05).After treatment,the incidence of adverse reactions of chemotherapy(vomiting,diarrhea and fatigue)in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Chaihu Zaoxiu Decoction combined with chemotherapy can improve the clinical efficacy and quality of life of patien
