摘  要:目的探讨Scc-Ag联合细胞角蛋白检测在鉴别诊断皮肤鳞状细胞癌与基底细胞癌中的应用价值。方法选择收治的皮肤鳞状细胞癌患者40例(SCC组),基底细胞癌患者40例(BCC组)。检测了2组患者血清和组织中鳞状上皮细胞癌抗原(squamous cell carinoma antigen,Scc-Ag)和细胞角蛋白19片段(cytokeratin 19 fragment,CYFRA21-1)水平变化。比较SCC-Ag及CYFRA21-1在2组患者血清中的阳性率。对比2种肿瘤标志物联合检测时对2种癌症诊断的灵敏度。结果SCC组患者血清及组织中SCC-Ag及CYFRA21-1的水平均高于BCC组,差异具有统计学意义(P<0.05)。2组患者血清中SCC-Ag及CYFRA21-1阳性表达率比较,差异无统计学意义(P<0.05)。2种肿瘤标志物联合检测时,与单独检测对比,对2种癌症的诊断灵敏度有明显提高,差异具有统计学意义(P<0.05)。结论联合检测SCC-Ag及CYFRA21-1水平可以提高皮肤鳞状细胞癌与基底细胞癌的临床诊断灵敏度,值得临床推广应用。Objective To investigate the value of Scc-Ag combined with cytokeratin in the differential diagnosis of cutaneous squamous cell carcinoma and basal cell carcinoma.Methods 40 patients with cutaneous squamous cell carcinoma admitted were selected as SCC group and 40 patients with basal cell carcinoma as BCC group.The levels of squamous cell carinoma antigen(Scc-Ag)and cytokeratin 19 fragment(CYFRA21-1)in serum and tissues of the two groups were examined.The positive rates of SCC-Ag and CYFRA21-1 in the serum of the 2 groups of patients were compared.The sensitivity of the 2 cancer markers in the combined detection of 2 tumor markers was also compared.Results The levels of SCC-Ag and CYFRA21-1 in serum and tissues of patients in SCC group were higher than those in BCC group,and the difference was statistically significant(P<0.05).There were no significant differences in serum levels of SCC-Ag and CYFRA21-1 between the 2 groups(P<0.05).When the two tumor markers were combined,the diagnostic sensitivity of the two cancers was significantly improved,and the difference was statistically significant(P<0.05).Conclusion The combined detection of SCC-Ag and CYFRA21-1 can improve the sensitivity of clinical differential diagnosis of cutaneous squamous cell carcinoma and basal cell carcinoma,and can be used for comprehensive judgment of various clinical information of patients.
