摘 要:目的探究Ki-67、p53的表达水平在不同分子分型的乳腺浸润性导管癌中的临床意义。方法采用免疫组化染色法检测159例单侧乳腺浸润性导管癌组织中Ki-67、p53表达水平,结合患者临床资料分析其与Ki-67、p53表达水平的相关性,并分析Ki-67与p53表达水平的相关性。结果Ki-67和p53在Luminal A型乳腺浸润性导管癌中的表达均显著低于其他亚型,其中在三阴性乳腺浸润性导管癌中表达最高(P<0.05)。乳腺浸润性导管癌患者Ki-67阳性表达与淋巴结转移、组织学分级、病理分期以及预后情况显著相关(χ^2=7.513,16.144,10.867,20.463,P<0.05);p53的阳性表达与组织学分级、病理分期以及预后情况显著相关(χ^2=13.325,9.680,23.020,P<0.05)。Ki-67的表达水平与p53的表达呈正相关关系(γ=0.397,P<0.01)。结论Ki-67、p53表达与乳腺浸润性导管癌分型相关,其高表达是预后不良因素;Ki-67表达水平与p53的表达呈正相关关系,为乳腺浸润性导管癌预后和靶向治疗提供了理论依据。Objective To explore the clinical significance of Ki-67,p53 expression in invasive ductal carcinoma of breast with different molecular types.Methods Immunohistochemical staining was used to detect the expression of Ki-67 and p53 in 159 cases of unilateral invasive ductal carcinoma of breast organization.The correlation between Ki-67 and p53 was analyzed with clinical data in combination with clinical data analysis of patients,and the correlation between Ki-67 and p53 was analyzed.Results The expression of Ki-67 and p53 in Luminal A type invasive ductal carcinoma with good prognosis were significantly lower than that in other subtypes with poor prognosis,and the expression of Ki-67 and p53 was the highest in triple negative invasive ductal carcinoma with the worst prognosis(P<0.05).The positive expression of Ki-67 was significantly correlated with lymph node metastasis,histological grade,pathological stage and prognosis(χ^2=7.513,16.144,10.867,20.463,P<0.05);the positive expression of p53 was significantly correlated with histological grade,pathological stage and prognosis(χ^2=13.325,9.680,23.020,P<0.05).Ki-67 expression level was positively correlated with p53 expression,and the correlation coefficient was 0.397,P<0.01.Conclusion The expression of Ki-67 and p53 are correlated with the classification of invasive ductal carcinoma of breast,and its high expression is a poor prognostic factor.The expression of Ki-67 is positively correlated with the expression of p53,which provide a theoretical basis for the prognosis and targeted therapy of invasive ductal carcinoma of breast.
