环磷酰胺联合顺铂治疗晚期卵巢癌的疗效及对患者血清CA125、HE4与Ki-67水平的影响

摘　　要：目的探讨环磷酰胺联合顺铂治疗晚期卵巢癌的疗效。方法选取70例晚期卵巢癌患者,其中32例患者(对照组)单纯使用顺铂治疗,观察组患者(n=38)使用顺铂联合环磷酰胺治疗。观察并比较两组患者的治疗效果。结果①两组患者血清指标(CA125、HE4、CCL-18)水平治疗前差异无统计学意义(P>0.05),上述指标值在观察组治疗后明显比对照组更低(P<0.05)。②两组患者血清指标(b FGF、VEGF、TNF-α)治疗前差异无统计学意义(P>0.05),上述指标值在观察组治疗后明显比对照组更低(P<0.05)。③两组患者肿瘤组织MVD、Ki-67、Survivin阳性率治疗前差异无统计学意义(P>0.05),上述抗原阳性率在观察组治疗后明显比对照组更低(P<0.05)。④观察组总有效率相较对照组明显更高(81.58%VS 53.13%,P<0.05)。⑤观察组治疗不良反应发生率相较对照组无差别(P>0.05)。结论环磷酰胺联合顺铂治疗晚期卵巢癌,能有效改善患者免疫功能,降低机体炎性反应,减少肿瘤组织血管生成,治疗效果显著且安全。因此,该方法值得应用与推广。Objective To study on the efficiency of cyclophosphamide combined with cis-platinum complexes in the treatment process of advanced ovarian cancer and the effect on the CA125,HE4 and Ki-67.Methods 70 patients with advanced ovarian cancer were selected,of which 32 patients(control group)were treated with cisplatin alone,and patients in the observation group(n=38)were treated with cisplatin combined with cyclophosphamide.Observe and compare the treatment effect of the 2 groups of patients.Results①Before treatment,the difference was not significant for groups about CA125,HE4 and CCL-18(P>0.05),after treatment,the index stated above was significantly lower than the control group(P<0.05).②Before treatment,the difference was not significant for groups about CA125,HE4 and CCL-18(P>0.05),after treatment,the index stated above was significantly lower than the control group(P<0.05).③There was no significant difference in the positive rate of MVD,Ki-67,Survivin between the 2 groups before treatment,but the positive rate of these antigens in the observation group was significantly lower than that of the control group after treatment(P<0.05).④The effective rate of the experimental group was higher than that of the control group(P<0.05).⑤There was no difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion Cyclophosphamide combined with cis-platinum complexes is used in the treatment of advanced ovarian cancer.It can effectively improve the immune function of patients,reduce the inflammatory reaction of the body,reduce the angiogenesis of tumor tissue,and the therapeutic effect is significant and safe.The treatment method is recommended in clinical.
