摘 要:目的探讨核磁共振(MRI)、血清巨噬细胞移动抑制因子(MIF)、微小RNA-1203(miR-1203)联合检测在肝癌早期诊断中的应用价值。方法收集60例肝癌患者为肝癌组,选取同期肝硬化患者60例为肝硬化组,另选同期进行体检的健康志愿者60例作为对照组。采用实时荧光定量聚合酶链反应(qRT-PCR)检测血清miR-1203的表达量;采用酶联免疫吸附试验(ELISA)检测血清MIF的水平。根据MIF、miR-1203表达量的平均值分为MIF高表达组(40例)、MIF低表达组(20例)、miR-1203高表达组(45例)、miR-1203低表达组(15例),比较分析MIF、miR-1203与肝癌患者临床病理参数的相关性;采用Pearson法分析血清MIF、miR-1203表达量的相关性。受试者工作特征(ROC)曲线分析血清MIF、miR-1203对肝癌的诊断价值。所有患者均进行影像学MRI检查。结果与对照组比较,肝硬化组与肝癌组患者血清MIF、miR-1203的表达量显著升高(P<0.05),且肝癌组显著高于肝硬化组(P<0.05);MIF与miR-1203呈正相关(γ=0.4175,P=0.0009)。MIF、miR-1203表达量与肝癌是否淋巴结转移、TNM分期、巴塞罗那分期密切相关(P<0.05)。MIF诊断肝癌的敏感度为48.33%,特异度为78.33%,AUC面积为0.581;miR-1203诊断肝癌的敏感度为63.33%,特异度为65.00%,AUC面积为0.662;MRI的敏感度为50.00%,特异度为83.33%,准确度为66.67%;MRI、血清MIF、miR-1203联合检测时敏感度为96.67%,特异度为75.00%,准确度为85.83%;联合检测敏感度显著高于单项检测(P<0.05)。结论肝癌患者血清MIF、miR-1203的表达量显著升高,MRI、MIF、miR-1203联合检测可提高肝癌诊断准确率。Objective To explore the value of combined detection of serum MIF and miR-1203 in early diagnosis of liver cancer.Methods 60 liver cancer patients were collected as liver cancer group,60 patients with liver cirrhosis in the same period were selected as liver cirrhosis group,and 60 healthy volunteers who underwent physical examination in the same period were selected as control group.qRT-PCR was used to detect the expression level of serum miR-1203.ELISA was used to detect the level of serum MIF.According to the average value of MIF and miR-1203 expression,it was divided into MIF high expression group(40 cases),MIF low expression group(20 cases),miR-1203 high expression group(45 cases),miR-1203 low expression group(15 cases),to compare and analyze the correlation between MIF,miR-1203 and clinicopathological parameters of liver cancer patients.Pearson method was used to analyze the correlation of serum MIF and miR-1203 expression levels.ROC curve analysis of the diagnostic value of serum MIF and miR-1203 for liver cancer.All patients underwent imaging MRI.Results Compared with the control group,the expression levels of serum MIF and miR-1203 in patients with liver cirrhosis and liver cancer were significantly increased(P<0.05),and the liver cancer group was significantly higher than that in liver cirrhosis(P<0.05).MIF was positively correlated with miR-1203(γ=0.4175,P=0.0009).The expression levels of MIF and miR-1203 were closely related to whether liver cancer had lymph node metastasis,TNM stage,and Barcelona stage(P<0.05).The sensitivity of MIF in the diagnosis of liver cancer was 48.33%,the specificity was 78.33%,and the AUC area was 0.581.The sensitivity of miR-1203 in the diagnosis of liver cancer was 63.33%,the specificity was 65.00%,and the AUC area was 0.662.The sensitivity of MRI was50.00%,the specificity was 83.33%,and the accuracy was 66.67%.MRI,serum MIF and miR-1203 combined detection had a sensitivity of 96.67%,a specificity of 75.00%,and an accuracy of 85.83%.The sensitivity of the combined test was
