摘　　要：目的探究二甲双胍联合吉西他滨治疗胰腺癌的疗效及对患者血清CA242水平的影响。方法选取86例胰腺癌患者,将患者随机分成2组,每组43例。对照组采用吉西他滨或者吉西他滨+顺铂(GP方案)进行治疗。观察组采用二甲双胍联合吉西他滨,或者联合GP方案进行治疗。观察比较2组患者的近期治疗效果,血清CA242水平,疾病无进展生存时间(PFS)和总生存期(OS)。结果观察组患者的疾病控制率(DCR)、总有效率(RR)与对照组无明显差异(P>0.05)。2组患者治疗后,观察组患者的血清CA242水平为(25.26±10.95)U/ml,明显低于对照组(47.31±11.63)U/ml,差异具有统计学意义(P<0.05)。观察组和对照组治疗后的不良反应及疾病无进展生存时间(PFS)无明显差异(P>0.05)。观察组患者的总生存期(OS)明显比对照组长,差异具有统计学意义(P<0.05)。结论二甲双胍联合吉西他滨化疗方案治疗胰腺癌,能够有效降低血清CA242水平,提高患者的总生存时间。Objective To explore the effects of metformin combined with gemcitabine chemotherapy on the efficacy,serum CA242 level and side effects of patients with pancreatic cancer.Methods 86 patients with pancreatic cancer were selected and randomly divided into 2 groups,43 cases in each group.The control group was treated with gemcitabine or gemcitabine plus cisplatin with GP,the observation group was treated with metformin combined with gemcitabine or combined with GP.The shortterm treatment effect of the 2 groups of patients,serum CA242 level,serum CA242 level,progression-free survival time(PFS)and total survival time(OS)were observed and compared.Results There was no significant difference in disease control rate(DCR)and total effective rate(RR)between the observation group and the control group(P>0.05).After treatment,the serum CA242 level in the observation group was(25.26±10.95)U·ml-1,significantly lower than that of the control group(47.31±11.63)U·ml-1,there had statistically significant difference(P<0.05).There was no significant difference in adverse reactions and disease progression free survival time(PFS)between the observation group and the control group(P>0.05).The total survival time(OS)of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The metformin combined with gemcitabine chemotherapy for pancreatic cancer can effectively reduce serum CA242 level and improve the total survival time of patients.
