大剂量奥美拉唑联合奥曲肽在消化性溃疡并上消化道出血治疗中的应用

摘　　要：目的研究大剂量奥美拉唑联合奥曲肽在消化性溃疡并上消化道出血的临床效果。方法选择2016年4月至2019年3月入院接受治疗的消化性溃疡并上消化道出血患者中的40例进行此次研究治疗,所有病患均为自愿参与。依据入院顺序分为对照组及观察组,各20例。两组患者均使用奥曲肽进行治疗,同时对照组使用常规剂量奥美拉唑,观察组则使用大剂量奥美拉唑进行治疗,对治疗效果进行比较。结果观察组治疗总有效率为95.0%,对照组为80.0%。差异显著有意义(P<0.05)。观察组输血量为(1.6±0.2)U、止血时间为(20.3±2.6)h、住院时间为(4.7±1.1)d,均明显低于对照组,差异显著有意义(P<0.05)。比较不良反应,差异不明显无意义(P>0.05)。结论对消化性溃疡并上消化道出血患者使用大剂量奥美拉唑以及奥曲肽进行治疗效果显著,可更快止血,减少输血量,且不会增加不良反应,应用价值较高。Objective To study the clinical effect of high-dose omeprazole combined with octreotide on peptic ulcer complicated with upper gastrointestinal hemorrhage.Methods 40 patients with peptic ulcer and upper gastrointestinal hemorrhage admitted for treatment from April 2016 to March 2019 were selected for this study.All patients participated voluntarily.According to the admission order,they were divided into control group and observation group,each with 20 cases.Both groups of patients were treated with octreotide,while the control group was treated with conventional dose of omeprazole,while the observation group was treated with large dose of omeprazole,and the therapeutic effects were compared.Results The total effective rate was 95.0%in the observation group and 80.0%in the control group.The difference was significant(P<0.05).The blood transfusion volume(1.6±0.2)U,hemostasis time(20.3±2.6)hours and hospitalization time(4.7±1.1)days in the observation group were significantly lower than those in the control group,with significant difference(P<0.05).Compared with adverse reactions,there was no significant difference(P>0.05).Conclusion Large-dose omeprazole and octreotide are effective in the treatment of peptic ulcer complicated with upper gastrointestinal hemorrhage.It can stop bleeding faster,reduce blood transfusion volume.and will not increase adverse reactions.It has high application value.
