摘　　要：目的:探讨在胃癌患者血浆中热休克蛋白90α(Hsp90α)的表达情况,以及与CEA、CA199、CA242在胃癌诊断中的临床价值。方法:检测207例胃癌患者血浆中Hsp90α及相关标志物的浓度水平。分析Hsp90α在患者性别、年龄、胃癌分期、发病部位及术前术后分组中的统计学差异。Hsp90α与CEA、CA199、CA242的相关系数。结果:胃癌组血浆中Hsp90α水平显著高于健康对照组,比较存在统计学差异(P<0.05)。胃癌患者血浆中Hsp90α水平在性别、年龄分组中比较无统计学差异(P>0.05);在分期分组中,高分期Hsp90α水平显著高于低分期,Ⅰ、Ⅱ与Ⅲ、Ⅳ期Hsp90α水平存在统计学差异(P<0.05);发病部位不同,Hsp90α水平胃淋巴瘤>胃体癌>贲门癌,组间比较有统计学差异(P<0.05);转移后Hsp90α水平显著高于未转移组,比较有统计学差异(P<0.05)。术前Hsp90α水平显著高于术后,比较有统计学差异(P<0.05)。Hsp90α与CEA、CA199、CA242的相关系数r=0.376、0.390、0.555(P<0.05),说明血浆中Hsp90α与CEA、CA199、CA242均存在正相关性,在胃癌中可作为辅助诊断指标。结论:血浆中Hsp90α水平在胃癌中显著升高,在胃癌的早期筛查、诊断、鉴别和治疗中具有重要临床参考价值。Objective:To investigate the expression of plasma heat shock protein 90α(Hsp90α)in gastric cancer and the clinical value in the diagnosis of gastric cancer.Methods:The blood concentrations of Hsp90αand related markers were detected in 207 patients with gastric cancer.The statistical differences of Hsp90αwere analysed in patients'gender,age,gastric cancer staging,site of onset,preoperation and postoperation.Results:The level of plasma Hsp90αin gastric cancer group was significantly higher than that in healthy control group,with statistical difference(P<0.05).There was no significant difference for plasma Hsp90αlevel in the gender and age groups of gastric cancer patients(P>0.05).In staging group,Hsp90αlevel in high stage was significantly higher than that of low stage,andⅠ,ⅡandⅢ,Ⅳstage Hsp90αlevel had statistical differences(P<0.05).Hsp90αlevel in gastric lymphoma>gastric body cancer>gastric cardia cancer,the difference between groups was statistically significant(P<0.05).The Hsp90αlevel in metastasis group was significantly higher than non-metastasis group,with statistically significant difference(P<0.05).Preoperative Hsp90αlevel was significantly higher than postoperative Hsp90αlevel,with statistically significant difference(P<0.05).The correlation between Hsp90αand CEA,CA199 and CA242 was r=0.376,0.390,0.555(P<0.05),indicating that the plasma Hsp90αlevel was positively correlated with CEA,CA199,CA242,and can be used as an auxiliary diagnostic indicator in gastric cancer.Conclusion:Hsp90αis significantly increased in gastric cancer,which has important clinical reference value in early screening,diagnosis,differentiation and treatment of gastric cancer.
