血清miRNA-130a表达水平对进展期非小细胞肺癌患者预后的预测价值

摘　　要：目的分析进展期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者血清微RNA-130a(miRNA-130a)的表达水平及其对患者预后的预测价值。方法将2017年1月至2019年1月哈尔滨医科大学附属肿瘤医院收治的110例进展期NSCLC患者纳入观察组,将该院同期56例健康体检者纳入对照组。比较两组研究对象血清miRNA-130a表达水平,并分析miRNA-30表达水平与进展期NSCLC患者预后的关系。结果观察组患者血清miRNA-130a表达水平显著高于对照组(P<0.001)。有淋巴结转移、肿瘤直径≥30 mm、临床分期为Ⅳ期、低分化、有多个远处脏器转移患者血清miRNA-130a表达水平均显著高于无淋巴结转移、肿瘤直径<30 mm、临床分期为Ⅲ期、中高分化、无多个远处脏器转移的患者(均P<0.05)。受试者操作特征曲线分析显示,血清miRNA-130a表达水平对进展期NSCLC患者预后有一定的预测价值,曲线下面积为0.829(95%CI:0.672~0.967,P=0.003),灵敏度为85.47%,特异度为60.06%。结论进展期NSCLC患者血清miRNA-130a高表达,且与预后密切相关,可作为评估预后的重要指标。Objective To analyze the expression level of serum microRNA-130a(miRNA-130a)in patients with advanced non-small cell lung cancer(NSCLC)and its prognostic value.Method From January 2017 to January 2019,110 NSCLC patients in Harbin Medical University Cancer Hospital were collected into observation group and 56 healthy people were collected into control group.The expression level of serum miRNA-130a between the two groups was compared,and the relationship between the expression level of miRNA-30 and the prognosis of patients with advanced NSCLC was analyzed.Result The expression level of serum miRNA-130a in observation group was significantly higher than that in control group(P<0.001).The expression level of miRNA-130a in patients with lymph node metastasis,tumor diameter≥30 mm,clinical stagingⅣ,poorly differentiation and multiple distant organ metastasis were significantly higher than those in patients without lymph node metastasis,tumor diameter<30 mm,clinical stagingⅢ,moderate and well differentiation,no multiple distant organ metastases(all P<0.05).The analysis of receiver operating characteristic curve showed that the expression level of serum miRNA-130a had a certain predictive value for the prognosis of patients with advanced NSCLC.The area under the curve was 0.829(95%CI:0.672~0.967,P=0.003),the sensitivity was 85.47%,and the specificity was 60.06%.Conclusion The high expression of serum miRNA-130a in patients with advanced NSCLC is closely related to the prognosis,it can be used as an important index to evaluate the prognosis.
