摘  要:目的分析乳腺浸润性导管癌(IDC)磁共振成像(MRI)影像组学特征与组织学分级的关系,并评价诊断效能。方法收集82例乳腺癌患者的表观弥散系数(ADC)和初始强化率(IER)两项MRI传统定量指标;手动分割动态增强磁共振成像(DCEMRI)图和ADC图上的乳腺癌病灶,对比分析低级别(1~11级)和高级别(Ⅲ级)IDC影像组学特征的差异,并评价诊断效能。结果高级别患者乳腺癌灶ADC值以及B sum variance、L energy、L_sum_average值均明显低于低级别IDC患者,B_uniform、L_SRE值均明显高于低级别IDC患者,差异均有统计学意义(P<0.01)。ADC图影像组学标签在鉴别IDC时具有较高的特异度、准确度、阴性预测值及阳性预测值,DCE图影像组学标签具有高灵敏度和低特异度。结论ADC值和乳腺MRI影像组学标签对鉴别IDC组织学分级具有一定意义,其中ADC图影像组学标签更具价值。Objective To analyze the relationship between the radiomic features of invasive ductal carcinoma(IDC)in magnetic resonance imaging(MRI) and the histological classification of breast cancer patients,and to determine the diagnostic efficacy.Method The clinical data of 82 patients with breast cancer were collected,from which the conventional quantitative measures of apparent diffusion coefficient(ADC) and initial enhancement rate(IER) were analyzed;manual segmentation was performed for the breast cancer lesions on dynamic contrast enhanced magnetic resonance imaging(DCE MRI) and ADC map,to compare and analyze the difference of radiomic features of low-grade(Ⅰ-Ⅱ) and high-grade(Ⅲ) IDC lesions,and then the diagnostic efficacy was evaluated.Result Compared with patients with low-grade IDC,the ADC values,B_sum_variance,L_energy and L_sum average values were lower in those with high-grade IDC lesions,and higher B_uniform and L_SRE values were observed in high-grade IDC versus low-grade ones(P<0.01).The specificity,accuracy,as well as negative and positive predictive values of radiomic tags in identifying IDC by ADC images were relatively higher,similarly,DCE imaging radiomic tags showed high sensitivity and low specificity in differentiating IDC.Conclusion ADC values and radiomic tags in breast MRI are of clinical significance in identifying the histological classification of breast IDC,and the radiomic tags of ADC imaging are more applicable.
