摘 要:目的探讨初诊多发性骨髓瘤(MM)患者并发高钙血症的影响因素。方法依据是否并发高钙血症将134例初诊MM患者分为高钙血症组(n=46)和非高钙血症组(n=88)。比较两组患者的性别、年龄、骨髓瘤骨病(MBD)分级、免疫分型、血肌酐(Scr)水平、β2微球蛋白(β2-MG)水平、白蛋白(ALB)水平、球蛋白(GLO)水平、乳酸脱氢酶(LDH)水平,采用Logistic回归模型分析初诊MM患者并发高钙血症的影响因素。结果高钙血症组MBD分级≥3级、Scr水平>177μmol/L、GLO水平>35g/L、β2-MG水平>3.5mg/L、LDH水平>245 IU/L的患者比例均明显高于非高钙血症组,差异均有统计学意义(P<0.01)。Logistic回归分析结果显示,MBD分级≥3级、Scr水平> 177μmol/L、GLO水平>35 g/L、β2-MG水平> 3.5 mg/L均是初诊MM患者并发高钙血症的独立危险因素(P<0.01)。结论初诊MM患者并发高钙血症与其MBD分级及Scr、GLO、β2-MG水平较高密切相关,检测上述指标对评估患者病情、指导临床治疗具有一定价值。Objective To analyze the factors influencing the occurrence of hypercalcemia in patients with newly diagnosed multiple myeloma(MM).Method The clinical data of 13 4 patients with newly diagnosed MM were retrospectively analyzed,the patients were divided into hypercalcemia group(n=46) and non-hypercalcemia group(n=88) according to the presence of hypercalcemia.The gender,age,myeloma bone disease(MBD) grade,immunophenotypes,serum creatinine(Scr),β2-microglobulin(β2-MG),albumin(ALB),globulin(GLO) and lactate dehydrogenase(LDH) were compared between the two groups,and the related factors affecting hypercalcemia in newly diagnosed MM patients were analyzed.Result The proportions of patients with MBD grade≥ 3,Scr>177 μmol/L,GLO>35 g/L,β2-MG>3.5 mg/L and LDH>245 IU/L in hypercalcemia group were significantly higher than those in non-hypercalcemia group(P<0.01).Logistic regression analysis showed that MBD grade≥ 3,Scr>177 μmol/L,GLO>35 g/L and β2-MG>3.5 mg/L were independent risk factors for hypercalcemia in patients with newly diagnosed MM(P<0.01).Conclusion Hypercalcemia in patients with newly diagnosed MM is closely related to the MBD grade and elevated levels of Scr,GLO and β2-MG.The detection of these indicators is clinically relevant in evaluating patients’ condition and guiding clinical treatment.
