高危型人乳头瘤病毒联合液基薄层细胞学检查在宫颈病变筛查中的应用效果

摘　　要：目的探讨高危型人乳头瘤病毒(HPV)联合液基薄层细胞学检查(TCT)在宫颈癌及宫颈高度病变中的筛查效果。方法选取3400例宫颈病变筛查者,所有患者均进行了HPV、TCT检查。结果 3400例宫颈病变筛查者中,HPV筛查的阳性率为15.35%,TCT筛查的阳性率为4.24%,HPV联合TCT筛查的阳性率为1.94%。HPV联合TCT筛查阳性者中宫颈上皮内瘤变(CIN)Ⅰ~Ⅲ级患者的比例高于单纯HPV、TCT筛查阳性者中CINⅠ~Ⅲ级患者的比例(P<0.05);单纯HPV筛查阳性者中CINⅠ~Ⅲ级患者的比例高于单纯TCT筛查阳性者中CINⅠ~Ⅲ级患者的比例(P<0.05)。年龄<50岁的宫颈病变患者单纯HPV、TCT筛查的阳性率均高于年龄≥50岁的宫颈病变患者(P<0.05)。不同年龄单纯HPV筛查阳性、单纯TCT筛查阳性、HPV联合TCT筛查阳性者中CINⅠ~Ⅲ级宫颈病变患者的比例比较,差异均无统计学意义(P>0.05)。结论 HPV联合TCT筛查宫颈癌及宫颈高度病变具有较高的应用价值,值得临床使用。Objective To investigate the screening effect of high-risk human papilloma virus(HP V) testing combined with thin-layer liquid-based cytology(TCT) detection in cervical cancer and high-grade cervical lesions.Method A total of 3400 cervical lesions screening subjects were selected,and all subjects were tested for both HPV testing and TCT detection.Result The positive rate of HPV screening was 15.35% while the positive rate of TCT screening was 4.24%,and the double positive rate of HPV combined TCT was 1.94% among the 3400 patients.The proportion of cervical intraepithelial neoplasia(CIN) Ⅰ-Ⅲ in HPV and TCT double positive cases was significantly higher than that in either HPV or TCT single positive cases(P<0.05).The proportion of CIN Ⅰ-Ⅲ in HPV single positive cases was significantly higher than that in TCT single positive cases(P<0.05).The positive rates of either HPV or TCT detection in women aged <50 were significantly higher than those whose aged≥ 50(P<0.05).There was no significant difference in the proportion of CIN Ⅰ-Ⅲ between patients with different age in HPV single positive,TCT single positive or HPV and TCT double positive group(P>0.05).Conclusion HPV testing combined with TCT detection indicate a great and potential application value for both cervical cancer and high-grade cervical lesions,which is recommended for further use in clinic context.
