益气养血汤加减对老年非小细胞肺癌患者术后化疗后血清肿瘤标志物及免疫水平的影响

摘　　要：目的探讨益气养血汤加减对老年非小细胞肺癌(NSCLC)患者术后化疗后血清肿瘤标志物及免疫水平的影响。方法选取2017年5月至2019年7月间南京大学医学院附属鼓楼医院收治的84例老年NSCLC患者,采用随机数表法分为研究组和对照组,每组42例。对照组患者采用常规化疗方案进行治疗,研究组患者在常规化疗基础上联合使用益气养血汤加减治疗,比较两组患者治疗前后血清肿瘤标志物及免疫水平的变化。结果研究组患者的肿瘤总控制率为83.3%,高于对照组的61.9%,差异有统计学意义(P<0.05)。两组患者治疗后癌胚抗原(CEA)、糖链抗原125(CA-125)和细胞角蛋白19片断(CYFRA21-1)水平不断降低,且研究组患者治疗6个月和12个月的CEA、CA-125和CYFRA21-1水平均低于对照组患者,差异均有统计学意义(均P<0.05)。两组患者治疗后CD3^+、CD4^+和CD4^+/CD8^+均不断升高,且研究组患者治疗6个月和12个月的CD3^+、CD4^+和CD4^+/CD8^+均高于对照组患者,差异均有统计学意义(均P<0.05)。治疗期间,两组患者均伴有便秘、腹泻、口腔炎、皮疹和呼吸困难等症状,但总发生率比较,差异无统计学意义(P>0.05)。结论益气养血汤加减治疗可有效促进老年NSCLC患者术后化疗后机体免疫功能的提高,降低血清肿瘤标志物水平,对患者生活质量的提高及降低NSCLC术后的复发和转移具有积极作用。Objective To investigate the effect of Yiqi Yangxue decoction on serum tumor markers and immunity levels in elderly patients with non-small cell lung cancer(NSCLC)after postoperative chemotherapy.Methods Eighty-four elderly NSCLC patients diagnosed and treated at Drum Tower Hospital Affiliated to Nanjing University School of Medicine from May 2017 to July 2019 were selected as research subjects.They were divided into a study group and a control group using random table method with 42 patients in each group.The study group was given Yiqi Yangxue decoction plus conventional chemotherapy and the control group was only given conventional chemotherapy.Changes in serum tumor markers and immunity levels were compared between the two groups before and after treatment.Results The overall tumor control rate was 83.3%in the study group which was higher than 61.9%of the control group(P<0.05).After the treatment,the levels of carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA-125)and human cytokeratin 21-1(CYFRA21-1)continuously reduced in the two groups,and the levels of them were lower in the study group than in the control group at 6 months and 12 months(all P<0.05).However,levels of CD3^+,CD4^+and CD4^+/CD8^+were continuously increased,and the levels of them were higher in the study group than in the control group at 6 months and 12 months(all P<0.05).In addition,during the treatment period,both groups were threatened by constipation,diarrhea,stomatitis,rash and dyspnea,and the difference in the overall incidence of complications was not statistically significant(P>0.05).Conclusion The addition and subtraction theory(AST)with Yiqi Yangxue decoction can effectively improve the immunity function,reduce the level of serum tumor markers,and have positive effects on the improvement of patients’quality of life and reduce of postoperative recurrence and metastasis in elderly NSCLC patients after postoperative chemotherapy.
