摘 要:目的通过锥形束CT研究安氏Ⅰ、Ⅱ、Ⅲ类错[牙合]畸形患者上下颌第一磨牙颊舌向倾斜度、基骨、牙槽骨、牙弓宽度的差异。方法选取错[牙合]畸形患者90例,利用Dolphin软件进行三维重建,测量不同错[牙合]畸形上下颌第一磨牙颊舌向倾斜度及横向距离。结果不同错[牙合]畸形上颌第一磨牙颊舌向倾斜度差别有统计学意义(P<0.05),上颌第一磨牙颊舌倾向斜度分别为右侧(2.33±3.36)°、(-3.04±5.43)°、(5.89±5.92)°,左侧(2.17±3.24)°、(-2.91±5.24)°、(5.29±5.48)°;下颌第一磨牙的颊舌向倾斜度安氏Ⅱ类相对其他错[牙合]较颊倾,差别有统计学意义(P<0.05);安氏Ⅲ类上颌基骨弓宽度(62.17±2.98)mm小于安氏Ⅰ类(64.41±3.56)mm,差别有统计学意义(P<0.05),安氏Ⅲ类下颌牙弓宽度(45.66±3.77)mm大于安氏Ⅰ类(43.92±2.39)mm、安氏Ⅱ类(43.51±3.07)mm,差别有统计学意义(P<0.05)。ANB角与上颌第一磨牙颊舌向倾斜度、下颌基骨弓宽度、下颌牙弓宽度成负相关,与下颌第一磨牙颊舌向倾斜度成正相关。结论不同错[牙合]畸形上下颌第一磨牙颊舌向倾斜度具有差异;安氏Ⅲ类患者上颌基骨弓相对狭窄,下颌牙弓相对偏大。Objective To study the differences of the buccolingual inclination,the widths of the basal bone,the alveolar bone and the intermolar widths between maxillary and mandibular first molars among patients with Angle ClassⅠ,Ⅱ,andⅢmalocclusion by cone-beam computed tomography(CBCT).Methods Ninety patients with malocclusion were selected and three-dimensional reconstruction was performed using Dolphin software to measure buccolingual inclination and transverse distances of the maxillary and mandibular first molars.Results The buccolingual inclination of the maxillary first molars is significantly different in different malocclusions(P<0.05).The right side buccolingual inclinations of the maxillary first molar in ClassⅠ,Ⅱ,Ⅲgroups were(2.33±3.36)°,(-3.04±5.43)°and(5.89±5.92)°,respectively,and the left side were(2.17±3.24)°,(-2.91±5.24)°,(5.29±5.48)°,respectively.The mandibular first molar had greater buccal inclination in ClassⅡ.The width of the maxillary basal bone in ClassⅢ(62.17±2.98)mm was narrower than that of ClassⅠ(64.41±3.56)mm,the difference was statistically significant(P<0.05).The mandibular arch at molar level in ClassⅢ(45.66±3.77)mm was larger than ClassⅠ(43.92±2.39)mm and ClassⅡ(43.51±3.07)mm,and the difference was statistically significant(P<0.05).The ANB angle was negatively correlated with the buccolingual inclination of the maxillary first molar,the width of the mandibular basal bone and the mandibular arch,and positively correlated with the buccolingual inclination of the mandibular first molar.Conclusions There are differences in the buccolingual inclination of the maxillary and mandibular in different malocclusions.The maxillary basal arch is relatively narrow and the mandibular arch is relatively large in Class Ⅲ patients.
