保乳联合前哨淋巴结活检治疗老年乳腺癌患者的疗效

摘　　要：目的探讨保乳联合前哨淋巴结活检治疗老年乳腺癌患者的疗效。方法选取2016年2月至2018年2月我院收治的76例老年乳腺癌患者,根据患者治疗意愿的不同将其分为观察组和对照组,每组38例。观察组给予保乳联合前哨淋巴结活检,对照组给予改良根治术治疗。比较两组手术时间、术中出血量、引流时间、总引流量及住院时间,统计两组患者满意度及并发症发生情况。结果观察组手术时间、引流时间、住院时间均短于对照组,术中出血量、总引流量均少于对照组,差异均有统计学意义(均P<0.05);观察组患者术后对乳房美观效果的满意度明显高于对照组患者,差异有统计学意义(P<0.05);观察组皮瓣坏死、皮下积液、患侧上肢水肿、脂肪液化等并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论通过前哨淋巴结活检可精准确定老年乳腺癌患者淋巴结状况,且前哨淋巴结活检术可缩短患者的手术时间,减少术中出血量及引流量,降低并发症的发生率。Objective To investigate the effect of breast-conserving combined with sentinel lymph node biopsy on elderly breast cancer patients.Methods A total of 76 elderly breast cancer patients admitted to our hospital from February 2016 to February 2018 were selected and divided into an observation group and a control group according to different treatment wishes,with 38 cases in each group.The observation group was given breast-conserving combined with sentinel lymph node biopsy,and the control group was given modified radical operation.The operation time,intraoperative blood loss,drainage time,total drainage volume and hospital stay of the two groups were compared,and the satisfaction and complications of the two groups were counted.Results The operation time,drainage time,and hospitalization time of the observation group were shorter than that of the control group,and the intraoperative blood loss and total drainage were less than the control group,and the differences were statistically significant(all P<0.05).Satisfaction with breast beauty effect of the observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05).The incidence of complications such as skin flap necrosis,subcutaneous effusion,affected upper limb edema,fat liquefaction and other complications of the observation group was lower than that of control group,the difference was statistically significant(P<0.05).Conclusion The sentinel lymph node biopsy can accurately determine the lymph node status of elderly breast cancer patients,and the sentinel lymph node biopsy can shorten the operation time of patients,reduce intraoperative blood loss and drainage,and reduce the incidence of various complications.
