单眼内直肌截除联合外直肌后徙术和双眼内直肌截除术治疗儿童集合不足型间歇性外斜视的疗效分析

摘　　要：目的探讨单眼内直肌截除联合外直肌后徙术与双眼内直肌截除术两种术式治疗儿童集合不足型IXT疗效差异.方法回顾性分析82例集合不足型IXT患儿的病例资料,采用两种术式对患儿进行治疗,单眼内直肌截除联合外直肌后徙45例,双眼内直肌截除术37例,针对术前和术后1年内不同时期的眼位和视功能情况进行统计学分析.结果单眼内直肌截除联合外直肌后徙组术后短期正位率更高(P<0.0001),但远期两组眼位变化无显著差别.单眼内直肌截除联合外直肌后徙组术后远期欠矫的比率增高(P=0.0171).双眼内直肌截除组术后短期易过矫(P<0.05),但远期过矫好转逐渐趋于正位.两组术后视功能均有好转.结论两种术式有不同的特点,根据病例的不同特点合理选择不同术式,术后效果均比较满意.Objective To evaluate the difference of curative effect between monocular medial rectus resection combined with lateral rectus recession and binocular medial rectus resection in treating children with convergence insufficiency intermittent exotropia.Methods The medical data of 82 children with convergence insufficiency exotropia were analyzed retrospectively,two surgical methods were used,45 cases had monocular medial rectus resection combined with lateral rectus recession,37 cases had binocular medial rectus resection.Statistical analysis was carried out on the eye position and binocular vision function before and after operation.Results The rate of short-term ocular position recovery was higher,which was used monocular medial rectus resection combined with lateral rectus recession(P<0.0001).However.there was no significant difference in eye position between the two groups.The rate of long-term undercorrection increased in the monocular medial rectus resection combined with lateral rectus recession group(P=0.0171).In the binocular medial rectus resection group,short-term postoperative overcorrection was easy(P<0.05).but the long-term overcorrection was getting better.In addition,binocular vision function improved in both groups.Conclusion The two kinds of operation methods have different characteristics,according to the characteristics of reasonable choice of operation methods,the postoperative results are satisfactory.
