摘 要:目的探讨胸部磁共振弥散加权成像技术在肺癌精确放疗靶区勾画中的临床价值。方法80例肺癌患者,在CT模拟定位当日行磁共振弥散加权成像技术(Diffusion weighted imaging,DWI)定位扫描,获取图像,于CT、DWI图像勾画大体肿瘤体积(gross tumor volume,GTV),设定放疗靶区,制定放疗计划,比较两种技术具体差异值。结果CT勾画GTV高于DWI,差异有统计学意义(P<0.05)。肺癌伴肺不张或(和)阻塞性肺炎者CT测得GTV值高于DWI(P<0.05);肺癌伴肺不张或(和)阻塞性肺炎者ADC值明显高于肿瘤组织(P<0.05)。结论胸部磁共振弥散加权成像技术可有效显示肿瘤体积,为精确放疗靶区勾画提供依据,临床价值高。Objective To investigate the Clinical value of thoracic magnetic resonance(MR)diffusion-weighted imaging(DWI)in delineating precise radiotherapy targets for lung cancer clinical value of thoracic diffusion weighted MR imaging in delineating precise radiotherapy targets for lung cancer.Methods Eighty patients with lung cancer underwent MR-DWI examination on the day of simulated CT localization to obtain the images.Gross tumor volume(GTV)was delineated according to CT and DWI images.Target areas were set up and radiotherapy scheme was instituted.The concrete difference value was compared between the two techniques.Results The GTV[(145.84±64.28)cm^3]depicted by CT was significantly higher than DWI[(102.61±58.73)cm^3](P<0.05).The value of GTV measured by CT in lung cancer patients with atelectasis or(and)obstructive pneumonia[(168.42±65.35)cm^3]was significantly higher than that of DWI[(115.71±59.76)cm^3](P<0.05).The value of ADC in lung cancer patients with atelectasis or(and)obstructive pneumonia[(2.51±0.50)s/mm^2]was significantly higher than that[(1.25±0.27)s/mm^2]in tumor tissues(P<0.05).Conclusion The thoracic MR diffusion weighted imaging technique can effectively display the volume of tumors to provide a basis for accurate target delineation of radiotherapy,which is of important clinical value.
