通腑活血汤对胃癌根治术后患者肠道黏膜屏障功能的影响

摘　　要：目的观察中药通腑活血汤对胃癌根治术患者肠道黏膜屏障功能的影响。方法选取2013年1月至2018年1月于成都中医药大学附属医院进行胃癌根治术的患者200例为研究对象,按照入院先后随机分为对照组和试验组,每组各100例。对照组给予早期肠内营养,试验组在对照组基础上给予通腑活血汤,比较两组患者肠道黏膜屏障功能[二胺氧化酶(DAO)、脂多糖(LPS)、降钙素原(PCT)、D-乳酸水平]、肠道功能恢复情况(肠鸣音恢复时间、肛门首次排气时间、肛门首次排便时间)。结果两组血清TP、ALB水平和MNA评分均明显升高(P<0.05),且试验组各指标均高于对照组(P<0.05);两组DAO、LPS、PCT、D-乳酸水平均明显降低(P<0.05),且试验组各指标均低于对照组(P<0.05);试验组肠鸣音恢复时间、肛门首次排气时间、肛门首次排便时间均短于对照组(P<0.05);两组疲劳症状评分均降低(P<0.05),躯体功能、角色功能和总体健康状况评分均升高(P<0.05),且试验组各指标评分优于对照组(P<0.05)。结论通腑活血汤可明显改善胃癌根治术患者营养状况,提升肠道黏膜屏障功能及生活质量。Objective To observe the effects of Tongfu Huoxue decoction on intestinal mucosal barrier function in patients after radical gastrectomy.Methods 200 cases of patients underwent radical gastrectomy in the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from January 2013 to January 2018 were randomly divided into the control group and the treatment group,with 100 cases in each group,according to the sequence of admission.The control group was given early enteral nutrition,and the treatment group was given Tongfu Huoxue decoction on the basis of the control group.Intestinal mucosal barrier function diamine oxidase(DAO),lipopolysaccharide(LPS),procalcitonin(PCT),D-lactate and intestinal function recovery(bowel sound recovery time,first anus exhaust time,first anus defecation time)were compared between the two groups.Results Serum TP,ALB levels and MNA scores were significantly increased in both groups(P<0.05),and each index in the treatment group was higher than that in the control group(P<0.05).The levels of DAO,LPS,PCT and D-lactate in both groups were significantly decreased(P<0.05),and each index in the treatment group was lower than that in the control group(P<0.05).The bowel sound recovery time,the first anus exhaust time and the first anus defecation time in the treatment group were all shorter than those in the control group(P<0.05).The scores of fatigue symptoms in both groups were reduced(P<0.05)while the scores of physical function,role function and overall health status were all increased(P<0.05),and the score of each index in the treatment group was better than that in the control group(P<0.05).Conclusion Tongfu Huoxue decoction can significantly improve the nutritional status of patients after radical gastrectomy,and improve the intestinal mucosal barrier function and quality of life.
