神经鞘瘤MRI影像诊断的多因素分析

摘　　要：目的分析神经鞘瘤患者MRI影像诊断的预测因素,为神经鞘瘤的MRI诊断提供依据。方法选取在龙泉驿区第一人民医院及重庆医科大学附属第一医院术前行MRI平扫及增强检查,且经手术证实为神经鞘瘤64例、非神经鞘瘤20例患者,对其MRI图像特征进行统计,并采用单因素及多因素Logistic回归分析得出神经鞘瘤MRI影像诊断的预测因素。结果神经鞘瘤术前MRI诊断准确率高。单因素分析结果显示,神经鞘瘤“形态、包膜完整、包膜下囊变、中央裂隙征、束状征、靶征、神经出入征、脂肪分离征”的MRI影像征象有统计学意义(P<0.05);多因素Logistic回归分析结果显示,神经鞘瘤“包膜完整、束状征及包膜下囊变”是神经鞘瘤MRI影像特征的独立预测因素(P<0.05)。结论神经鞘瘤MRI影像评价中,神经鞘瘤具有多个影像征象特征,其中神经鞘瘤的“包膜完整、束状征及包膜下囊变”3个征象是独立预测征象,认识这些独立预测征象,可进一步提高神经鞘瘤的术前诊断符合率。Objective To analyze the predictive factors of magnetic resonance imaging(MRI)features in patients with schwannoma and provide the basis for diagnosing neurilemoma by MRI.Methods The MRI features of 64 patients with neurilemoma and 20 patients with non-neurilemoma who underwent the MRI plain scan and enhanced examination before operation and diagnosed by operation in The First People's Hospital of Longquanyi District and The First Affiliated Hospital of Chongqing Medical University were collected to analyze the predictive factors of the MRI features of neurilemoma by the Univariate and multivariate Logistic regression.Results The preoperative diagnosis of neurilemoma by MRI is highly accurate.The results of the univariate analysis showed that the MRI features of neurilemoma including intact morphology and capsule,subcapsular cystic change,central fissure sign,fascicular sign,target sign,nerve access sign and fat separation sign were statistically significant(P<0.05).The results of the multivariate Logistic regression analysis showed that intact capsule,fascicular sign and subcapsular cystic change were the independent predictive factors of the MRI features of neurilemoma(P<0.05).Conclusion In the MRI imaging evaluation of neurilemoma,there are multiple imaging features among which the three signs including intact capsule,fascicular sign and subcapsular cystic change are the independent predictive factors of the MRI features of neurilemoma,which is beneficial to further improve the accuracy of the preoperative diagnosis of neurilemoma.
