CDT结合压力袖套治疗对AIS偏瘫上肢水肿患者的恢复情况及FMA评分、Barthel指数的影响

摘　　要：目的手法淋巴引流综合治疗(CDT)结合压力袖套治疗对急性缺血性脑卒中(AIS)偏瘫上肢水肿患者的恢复情况及Fugl-Meyer评定量表(FMA)评分、Barthel指数的影响。方法选取120例AIS偏瘫上肢水肿患者,按照随机数字表分为观察组和对照组,每组60例。两组患者均采取CDT治疗,观察组患者在此基础上联合压力袖套治疗。比较两组患者的水肿情况、体重指数、肢体周径、治疗优良率、FMA评分、Barthel指数以及治疗满意度的差异。结果经过治疗后,两组患者的水肿情况、体重指数以及肢体周径明显下降,差异均有统计学意义(t分别=5.04、6.79、12.93;1.95、5.84、6.67,P均<0.05),且观察组患者的水肿情况、体重指数以及肢体周径明显低于对照组,差异均有统计学意义(t分别=3.21、11.49、8.88,P均<0.05),观察组患者的FMA评分、Barthel指数明显高于对照组(t分别=8.53、7.73,P均<0.05),观察组患者的治疗优良率和满意度明显高于对照组,差异均有统计学意义(χ2分别=4.18、9.72,P均<0.05)。结论CDT结合压力袖套治疗AIS偏瘫上肢水肿患者治疗效果显著,可明显改善局部水肿情况,有效改善患者的FMA评分、Barthel指数,对于患者的预后具有积极的意义。Objective To explore the effect of CDT combined with pressure cuff on the recovery,the score of FMA and Barthel index.of AIS hemiplegic patients with upper extremity edema.Methods Totally 120 patients with hemiplegic upper extremity edema were selected as the study objects and randomized into the observation group and the control group,60 patients in each group.Both groups were treated with CDT,and the observation group was treated with pressure cuff.The differences in edema,body mass index,limb circumference,curative efficiency,FMA score,Barthel index and treatment satisfaction between the two groups were compared.Results After treatment,the edema,body mass index,and limb circumference of the two groups decreased significantly(t=5.04,6.79,12.93,1.95,5.84,6.67,P<0.05).The edema,body mass index and limb circumference of the observation group were significantly lower than those of the control group(t=3.21,11.49,8.88,P<0.05),while the FMA score and Barthel index of the observation group were significantly higher than those of the control group(t=8.53,7.73,P<0.05)as well as the curative efficiency and satisfaction(χ2=4.18,9.72,P<0.05).Conclusion CDT combined with pressure cuff can effectively improve the FMA and Barthel index of AIS hemiplegic patients with upper extremity edema by improving the local edema,which has positive significance for the prognosis of patients.
