摘　　要：目的:评估赝复体修复单侧上颌骨缺损即刻及1月后咀嚼功能变化。方法:选择因一侧上颌骨缺损就诊制作支架中空式永久性赝复体修复的患者为研究对象。采用吸光光度法测定赝复体初戴前后及戴用1月后的咀嚼效率;T-scan Ⅱ咬合分析仪测定患者咬合加载时间(MLT)及咬合力分布状况。结果:赝复体初戴前后咀嚼效率吸光光度平均值分别为0.403abs和0.338abs,无统计学差异(P>0.05),戴用一月后咀嚼效率吸光光度平均值提高至0.500abs,同初戴前后相比差异均有统计学意义(P<0.01)。赝复体初戴前后及一月后咬合加载时间、COFY值差异无统计学意义(P>0.05);赝复体戴用1月后咬合力中心(COF)向缺损侧偏移约5.72mm,缺损区可承受约18.46%的咬合力,同初戴前相比差异均有显著统计学意义(P<0.01),但同初戴后相比差异无统计学意义(P>0.05)。结论:赝复体戴用1月后显著改善单侧上颌骨缺损患者的咀嚼功能。Objective:To evaluate the masticatory function of the patients with unilateral maxillary defect restored by obturator prosthesis in different periods(before maxillofacial prosthetic treatment,after the treatment immediately and 1 month later).Methods:Patients with unilateral maxillary defect restored by prosthesis were recruited in this study.The masticatory efficiency was measured by spectrophotometer.The distribution of occlusal force was measured by T-ScanII Computerized Occlusal Analysis system.Results:There was no statistically significant difference in mastication efficiency before and after the initial wearing of prosthesis(P>0.05).The mastication efficiency improved significantly one month after wearing of prothesis(P<0.01).For the distribution of occlusal force with prosthesis,there were no significant difference in the maximum-load time(MLT)and center of force on the Y axis(COFY)(P>0.05).One month after wearing of prothesis,the COF moved 5.72mm to the defective side and the defective side could bear 18.46%occlusal force on average(P<0.01).But compared with initial wearing of prosthesis,there was no significant difference(P>0.05).Conclusion:The masticatory function of the patients with unilateral maxillary defect was significantly improved by wearing obturator prosthesis for one month.
