摘  要:目的:讨论不同头位和椅位前后向倾斜角度对下颌位置的影响。方法:选择拥有完整牙列的18位临床实习生作为研究对象,调整被试验者头部前后倾角,设定3种头位:前倾15°(H-15),正常(H0),后仰15°(H+15),调整牙椅椅背与垂直面的倾角,设定3种椅位:后倾30°(C30)、后倾45°(C45)、后倾60°(C60)。将不同头位椅位条件下前牙区的口内三维扫描数据与牙尖交错位的数据拟合配准,测量9种不同头位椅位下切牙颈缘最低点的偏移量,进行统计分析。结果:比较各头位椅位条件下的下颌偏移量均数,C60大于C30和C45,相同椅位下,H0时较小,但各头位与椅位间无显著差异(P>0.05)。C60时各头位下颌偏移量变异系数的均数显著大于C30和C45时的(P<0.05),后两者之间无显著差异(P>0.05),C45H0的组合时变异系数较小。结论:不同头位和椅位影响患者下颌位置的可重复性,以及与牙尖交错位的一致性。在进行与咬合关系记录及调整相关的临床操作时,采用正常头位及45°椅位较为适宜。Objective:To investigate the influence of different head and chair position on the position of the mandible.Methods:Eighteen interns with complete dentition(8 males,10 females,mean age 21.0±1.1 years old)were selected for this study,the head tilt angle of the subjects was adjusted by using the pillow of the dental chair.Three types of head positions were set:forward tilt 15 degrees(H-15),normal(H0),backward tilt 15 degrees(H+15).adjust the angle of inclination of the back of the chair and the vertical plane,set 3 kinds of position:backward 30°(C30),backward 45°(C45),backward 60°(C60).The intra-oral three-dimensional scan data of the anterior region under different head position conditions were fitted to the intercuspal interdigitation data,and the offset of the lowest point of the incisor neck under 9 different head positions was measured for statistical analysis.Results:The mean distance between the neck position of incisor and S0 was compared.C60 was larger than C30 and C45,while H0 was smaller in the same chair position,but there was no significant difference between the head position and the chair position(P>0.05).The mean coefficient of variation of head position at C60 was significantly greater than that at C30 and C45(P<0.05),and there was no significant difference between the latter two groups(P>0.05).The coefficient of variation of C45H0 combination was smaller.Conclusions:Different head and chair position affect the repeatability of mandible position and the consistency of intercuspal position.In the clinical treatment related to the record and adjustment of occlusal relationship,the adjustment of anterior and posterior angle of patients'head and chair position should be considered,and the normal head position with 45°chair position is better.
