探讨血清tPSA和fPSA及f/tPSA预测前列腺腺癌Gleason评分的价值

摘　　要：目的:探讨前列腺腺癌患者血清总前列腺特异抗原(tPSA)、游离前列腺特异抗原(fPSA)和f/tPSA三项指标预测Gleason评分的价值。方法:回顾性分析123例经直肠行前列腺穿刺病理诊断确诊为前列腺腺癌(PA)患者,以前列腺腺癌Gleason评分进行分组,依次为A组(6分)、B组(7分)、C组(8分)、D组(9分)。收集患者血清tPSA,fPSA,f/tPSA三项数据进行统计学分析。结果:123例前列腺腺癌患者A组17例,B组57例,C组16例,D组33例。123例患者血清tPSA,fPSA,f/tPSA(%)分别为(68.12±33.65) ng/mL,(5.44±5.44) ng/mL,(8.34±5.77)%,比较正常体检者(NC)(100例)和良性前列腺增生(BPH)症患者(100例)的三项指标,差异有统计学意义(P<0.05)。Gleason评分6分前列腺腺癌tPSA预测值为(32.60±31.93) ng/mL,f/tPSA预测值为(14.52±5.54)%。将前列腺腺癌Gleason评分6分的血清tPSA和f/tPSA预测值分别与7分、8分、9分的预测值进行比较,差异有统计学意义(P<0.05)。将前列腺腺癌Gleason评分9分的f/tPSA预测值分别与6分、7分、8分的预测值进行比较,差异有统计学意义(P<0.05)。结论:联合血清tPSA及f/tPSA预测值可较好预测前列腺腺癌Gleason评分6分,f/tPSA预测值可较好预测前列腺癌Gleason评分9分。Objective:To explore the value of serum total prostatic specific antigen(tPSA),free prostate specific antigen(fPSA) and f/tPSA(%) in predicting Gleason score in patients with prostate adenocarcinoma.Methods:Retrospective analysis was performed on 123 cases of prostate adenocarcinoma diagnosed by pathological diagnosis of prostate biopsy through rectum.The Gleason score(prognostic) of prostate adenocarcinoma was divided into group A(6 points),group B(7 points),group C(8 points) and group D(9 points).Serum tPSA,fPSA and f/tPSA(%) were collected for statistical analysis.Results:All 123 cases of prostate adenocarcinoma in group A(17 cases),group B(57 cases),group C(16 cases) and group D(33 cases).The mean values of serum tPSA,fPSA and f/tPSA(%) of 123 patients were(68.12± 33.65) ng/mL,(5.44±5.44) ng/mL,(8.34±5.77)%,Comparing the three indicators with normal physical examination(NC)(100 cases) and benign prostatic hyperplasia(BPH) patients(100 cases),the difference was statistically significant(P<0.05).Gleason score of 6 indicates that the tPSA of prostate adenocarcinoma is(32.60±31.93) ng/mL,and f/tPSA(%) is(14.52 ±5.54)%.The serum tPSA and f/tPSA(%) predicted by Gleason score of 6 were compared with the predicted values of 7,8 and 9 points respectively,and the difference was statistically significant(P<0.05).The predictive value of f/tPSA(%) of prostate adenocarcinoma Gleason score 6 was com-pared with the predictive value of 7,8 and 9 points respectively,and the difference was statistically significant(P<0.05).Conclusion:The combination of serum tPSA and f/tPSA predicted a better Gleason score of 6 for prostate adenocarcinoma.f/tPSA(%) predicted prostate cancer Gleason score of 9.
