摘 要:目的探讨声触诊组织成像定量技术(VTIQ)与组织弹性成像技术(ETE)诊断微小乳腺癌的价值.方法对78例(97个结节)患者最大直径≤1cm的乳腺结节在麦默通旋切术或手术切除前分别采用VTIQ及ETE二种超声弹性成像方法测量其平均剪切波速度(SWVm)、弹性面积与二维声像图面积的比值(AR)作为硬度参数.以病理检查为金标准,根据结节良恶性结果绘制受试者操作特性曲线(ROC),确定二种方法诊断微小乳腺癌的阈值并分别探讨诊断灵敏度、特异性及准确性.结果97个结节中,良性58个,恶性39个,恶性组结节SWVm及AR大于良性组结节(P<0.001).VTIQ及ETE二种方法ROC曲线下面积分别为0.896、0.818,当诊断阈值分别为3.5m/s、1.21时约登指数最大,诊断灵敏度分别为89.74%、74.36%,特异性分别为84.48%、74.14%,准确性分别为86.60%、74.23%,χ^2检验灵敏度、特异性及准确性的差异,P=0.003、0.147、0.020,灵敏度及准确性差异有统计学意义,特异性差异无统计学意义.结论VTIQ与ETE均可作为临床诊断微小乳腺癌的可行方法,VTIQ比ETE具有更高的灵敏度与准确性.Objective To evaluate the value of virtual touch tissue imaging quantification(VTIQ)and esie touch elasiricity imaging(ETE)in diagnosis of micro breast cancer.Methods The mean shear wave velocity(SWVm)and the ratio of elastic area to two dimemional image area(AR)were measured by VTIQ and ETE before Mammotome or surgical operation for 97 breast nodules with the largest diameter^lcm in 78 patients.Taking pathology as gold standard,according to the results of benign and malignant nodules,the operating characteristic curve(ROC)of the subjects was drawn,the sensitivity,specificity and accuracy of the diagnosis and the threshold value of the two methods for the diagnosis of micro breast cancer were determined.Results Among 97 nodules,58 nodules were benign,39 nodules were malignant,SWVm and AR.in malignant group were larger than those in benign group(P<0.001).The area under ROC curve ofVTIQ and ETE were 0.896 and 0.818,respectively.When the diagnostic threshold were 3.5m/s and 1.21,the Youden index was the largest,the diagnostic sensitivity were 89.74%and 74.36%,the specificity were 84.48%and 74.14%,and the accuracy were 86.60%and 74.23%,respecrively.The difference of sensitivity,specificity and accuracy of chi-square test were 0.003,0.147 and 0.020,respectively.The difference of sensitivity and accuracy were statistically significant,but the difference of specificity was not statistically significant.Conclusion VTIQ and ETE can be used as feasible methods for clinical diagnosis of micro breast cancer.VTIQ has higher sensitivity and accuracy than ETE.
