摘　　要：目的探讨血清人附睾蛋白4(HE4)和血浆D-二聚体检测在卵巢恶性肿瘤诊断中的应用价值。方法测定176例卵巢癌患者和92例卵巢良性肿瘤患者外周血中HE4和D-二聚体的表达水平,比较HE4和D-二聚体在各组之间的表达差异,应用Logistic回归和ROC曲线分析其诊断价值。结果 HE4和D-二聚体在卵巢恶性肿瘤患者中的水平明显高于卵巢良性肿瘤患者(P <0.05);在卵巢恶性肿瘤晚期患者明显高于早期患者(P <0.05);在浆液性卵巢恶性肿瘤中明显高于非浆液性卵巢恶性肿瘤(P <0.05)。HE4和D-二聚体联合检测可以提高对卵巢癌的诊断效能。结论联合检测血清HE4和血浆D-二聚体有助于鉴别卵巢良性、恶性肿瘤,提高诊断效能,争取早期发现与治疗。Objective To investigate the diagnostic value of combined detection of human epididymis protein(HE4)and plasma D-dimer in ovarian cancer.Methods The serum HE4 and plasma D-dimer were detected in 176 ovarian cancer patients and 92 benign ovarian tumor patients.Logistic regression and ROC curve analysis were used to assess the diagnostic value of various indicators.Results The levels of serum HE4 and plasma D-dimer in ovarian cancer patients were significantly higher than those in ovarian benign tumor patients;the levels of serum HE4 and plasma D-dimer in advanced malignant ovarian tumor patients were significantly higher than that of patients with early malignant ovarian tumor;the levels of plasma D-dimer in serous ovarian cancer were also significantly higher than that of patients without serous ovarian cancer(P<0.05).Combined detection of serum HE4 and plasma D-dimer can improve the diagnostic efficacy of ovarian malignant tumors.Conclusion Combined detection of serum HE4 and plasma D-dimer can help to distinguish benign ovarian diseases and ovarian cancer,raise the diagnostic value and strive for early diagnosis and early treatment.
