摘 要:目的探讨增强磁共振成像(MRI)联合血清糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、癌胚抗原(CEA)检测对胰腺癌诊断准确性的影响。方法选择2015年3月-2019年3月本院收治的123例胰腺癌患者作为研究组,同期选择本院120例作为健康对照组;2组人员均接受增强MRI检查,并经电化学发光法对血清CA19-9、CA125、CEA水平进行检测;分析增强MRI、血清肿瘤标志物单独诊断、联合诊断灵敏度、特异度及准确性。结果增强MRI扫描胰腺癌发现:胰腺肿块呈T1WI低信号或较低信号,T2WI呈混杂信号或较高信号;在肿瘤浸润胰周时,周围脂肪呈高信号,并伴低信号条纹状影;研究组患者血清CA19-9、CA125、CEA水平均高于健康对照组(P <0.05);增强MRI联合血清CA19-9、CA125、CEA诊断胰腺癌的灵敏度、准确性均高于单项诊断(P <0.05);增强MRI联合血清CA19-9、CA125、CEA诊断胰腺癌的特异度与单项诊断相比,差异无统计学意义(P> 0.05)。结论开展增强MRI联合血清CA19-9、CA125、CEA检测能使胰腺癌诊断灵敏度、特异度及准确性提高,减少误诊及漏诊,为临床早期治疗提供依据。Objective To investigate the effect of enhanced MRI combined with Serum CA19-9(glucose antigen 19-9),CA125(glucose antigen 125)and CEA(carcinoembryonic antigen)on the diagnostic accuracy of pancreatic cancer.Methods Totally 123 cases of pancreatic cancer admitted to our hospital from March 2015 to March 2019 were selected as the study group.In the same period,120 cases of healthy check volunteers were selected as the control group.Both of the two groups received enhanced MRI and the detection of the levels of serum CA19-9,CA125 and CEA by electrochemiluminescence to analyze enhanced MRI,serum tumor markers alone,combined diagnostic sensitivity,specificity and accuracy.Results It is found in the enhanced MRI scan of pancreatic cancer:pancreatic mass showed T1WI low signal or lower signal,T2WI showed mixed signal or higher signal;the enhanced MRI scan showed that the pancreatic mass showed a low signal or a low signal on T1WI,and a mixed signal or a higher signal on T2WI.The sensitivity and accuracy of enhanced MRI combined with serum CA19-9,CA125,CEA in the diagnosis of pancreatic cancer were higher than the single diagnosis,with the differences statistically significant(P<0.05).The specificity of enhanced MRI combined with serum CA19-9,CA125,CEA in the diagnosis of pancreatic cancer was higher than that in single diagnosis,but the difference was not statistically significant(P>0.05).Conclusion The combination of enhanced MRI combined with serum CA19-9,CA125 and CEA can improve the sensitivity,specificity and accuracy of pancreatic cancer diagnosis,reduce misdiagnosis and miss diagnosis,and provide evidence for the early clinical treatment.
