儿童恒前牙外伤后牙髓预后及相关影响因素分析

摘　　要：目的:探讨儿童恒前牙外伤后牙髓预后及相关影响因素。方法:选择2018年3-11月我院口腔科收治的110例儿童恒前牙外伤患儿(145颗患牙)作为对象,根据患儿的牙损伤程度实施不同方式的治疗,所有患儿治疗后随访1年,评估其牙髓预后情况,对影响牙髓预后因素进行单因素及多因素Logistic回归分析。结果:110例患儿的145颗牙齿中共121颗牙髓存活,其总存活率为83.45%,其中24颗发生坏死,坏死率为16.55%。单因素分析结果发现,影响牙髓预后因素中牙根发育情况以及外伤类型与患儿的牙髓预后相关,差异具有统计学意义(P<0.05);多因素Logistic回归分析发现,牙齿脱位以及牙根发育完全是造成牙髓坏死的独立危险因素(P<0.05)。结论:根据儿童不同情况的恒前牙外伤实施针对性治疗,牙髓预后较好,其中牙齿脱位以及牙根发育完全是造成牙髓坏死的独立危险因素。Objective To explore the prognosis and related factors of pulp after permanent anterior tooth trauma in children.Methods110 children with permanent anterior tooth trauma(145 teeth)from March 2018 to November 2018 in our hospital Department of Stomatology were selected as the research objects.According to the degree of tooth injury,all children were treated in different ways.All children were followed up for one year after treatment to evaluate the pre and post pulpal condition,and single and multi factor Logistic regression analysis was conducted to analyze the factors affecting the pulp prognosis.Results Of the 145 teeth in 110 children,121 teeth survived,with a total survival rate of 83.45%.Among them,24 teeth were necrotic,with a necrosis rate of 16.55%.The Results of single factor analysis showed that the development of root and the type of trauma were related to the prognosis of pulp,the difference was statistically significant(P<0.05).The Results of multi factor Logistic regression analysis showed that the avulsed tooth and the development of root were the independent risk factors of pulp necrosis(P<0.05).Conclusions According to the different conditions of children's permanent tooth trauma,the prognosis is better after targeted treatment,among which avulsed tooth and root development are independent risk factors for pulp necrosis.
