多层螺旋CT评估肾癌病理分级的可行性分析

摘　　要：目的本文探究64排螺旋CT评估肾癌病理分级的可行性,为临床应用提供理论依据。方法将2015年2月至2016年6月共纳入符合标准的透明细胞肾癌患者34例。经手术或肾穿刺活检确定病理分期。术前行64排螺旋CT检查。观察病灶形态学特征,有无瘤栓,以及影像学指征。结果经组织病理学分析可知,Ⅰ级患者有6例,Ⅱ级患者有14例,Ⅲ级患者有10例,Ⅳ级患者有4例。不同病理级别的患者的年龄、性别差异较小,无统计学意义(P>0.05)。不同病理学分级的癌灶最大径、囊变坏死、钙化、浸润情况及有无瘤栓存在明显差异(P<0.05)。各病理级别平扫CT值及相对密度指数差别较小(P>0.05)。而皮质期及髓质期扫描,影像学指标:CT值、CT差值、增强百分比、增强指数,在不同病理级别有统计学差异(P<0.05)。结论 64排螺旋CT用于评估肾癌病理分级具有一定的可行性,值得在临床广泛推广应用。Objective To explore the feasibility of 64 slice spiral CT in evaluating the pathological grading of renal cell carcinoma,and to provide theoretical basis for clinical application.Methods a total of 32 cases of clear cell renal cell carcinoma were enrolled from February 2015 to June 2016.Pathologic staging was confirmed by surgery or biopsy of kidney.64 slice spiral CT examination before operation.Morphological characteristics of the lesions,there are no tumor thrombosis,as well as imaging indications.Results the histopathological analysis showed that there were 6 cases of grade I patients,14 cases of grade II patients,10 cases of grade III patients,2 cases of grade IV patients.The age and sex of the patients with different pathological grades were small,and there was no significant difference(P>0.05).The maximum diameter,cystic necrosis,calcification,infiltration and tumor thrombus in different pathological grades were significantly different(P<0.05).The difference of CT value and relative density index(P>0.05)was small in all pathological grades.In the cortical and medullary phase,the higher the pathological grade,the difference of CT value,the CT value,the enhancement percentage,and the enhancement index were significantly reduced(P<0.05).Conclusion 64 slice spiral CT can be used to evaluate the pathological grading of renal cell carcinoma.It is worth to be widely used in clinical practice.
