摘  要:目的:了解负性共刺激分子T细胞免疫球蛋白黏蛋白分子-3(Tim-3)和半乳糖凝集素-9(Galectin-9)在不同病变的子宫颈脱落细胞中的表达情况,以探讨负性共刺激分子与子宫颈癌发生、发展的关系。方法:收集2015年8月至2015年10月于西安交通大学第一附属医院妇产科就诊患者子宫颈脱落细胞标本84例。其中正常子宫颈(NC)17例、低级别鳞状上皮内病变(LSIL)22例、高级别鳞状上皮内病变(HSIL)25例、子宫颈鳞状细胞癌(CC)20例。免疫细胞化学检测Tim-3、Galectin-9蛋白在子宫颈脱落细胞中的表达。结果:免疫细胞化学检测显示Tim-3在NC和异常子宫颈脱落上皮细胞的胞核和胞浆中均有表达,其在NC、LSIL、HSIL、CC中的表达率分别为82.35%、59.09%、52.00%、30.00%,HSIL和CC与NC相比差异有统计学意义(P<0.05)。Galectin-9表达于NC和异常子宫颈脱落上皮细胞的胞核中,NC、LSIL、HSIL、CC中的表达率分别为23.53%、36.36%、60.00%、85.00%,HSIL和CC与NC相比差异有统计学意义(P<0.05)。在人乳头瘤病毒(HPV)感染的子宫颈脱落细胞中,Tim-3呈高表达(P=0.000),Galectin-9(P=0.015)呈低表达。结论:在NC、LSIL、HSIL、SCC的脱落细胞中Tim-3表达逐级降低、Galectin-9表达逐级增高,这一表达模式可能与子宫颈病变进展和HPV感染有关。Objective:To investigate the expression pattern of Tim-3 and Galectin-9 in different lesions of cervical cytological specimens,and understand the role of negative costimulatory molecules in cervical lesions progression.Methods:84 cervical cytological samples were collected from the Department of Obstetrics and Gynecology in The First Affiliated Hospital of Xi′an Jiaotong University from August 2015 to October 2015,including normal cervix(NC=17 cases),low-grade squamous intraepithelial lesions(LSIL=22 cases),high-grade squamous intraepithelial lesions(HSIL=25 cases)and squamous cell carcinoma(CC=20 cases).The expression of Tim-3 and Galectin-9 were detected by using immunocytochemistry.Results:Tim-3 was expressed in the nucleus and cytoplasm of normal and abnormal exfoliated cervical epithelial cells.The positive expression rate of Tim-3 in NC,LSIL,HSIL and CC were 82.35%,59.09%,52.00%and 30.00%,respectively.Compared with NC,HSIL and CC are statistically significant(P<0.05).Galectin-9 was expressed in the nucleus of normal and abnormal exfoliated cervical epithelial cells.The positive expression rate of Galectin-9 was 23.53%in NC,36.36%in LSIL,60.00%in HSIL and 85.00%in CC.Compared with NC,Galectin-9 expression in HSIL and CC were statistically significant(P<0.05).In the cervical exfoliated cells of human papilloma virus(HPV)infection,Tim-3 hada higher expression(P=0.000)while Galectin-9 had a lower expression(P=0.015).Conclusions:The decreased expression of Tim-3 and increased expression of Galectin-9 might be associated with the progression of cervical lesion and HPV infection.
