玻璃体切除硅油填充术后早期高眼压对角膜内皮细胞的影响

摘　　要：目的观察玻璃体切除硅油填充术后早期高眼压对增生性糖尿病性视网膜病变(PDR)患者角膜内皮细胞的影响。方法回顾性病例对照研究。大连大学附属中山医院2018年1月至2019年3月玻璃体切除硅油填充术60例(60只眼),其中术后发生早期高眼压者30例(30只眼,A组),术后眼压正常者30例(30只眼,B组)。两组进行对比分析。结果术前两组眼压、角膜内皮细胞密度、细胞多形性变异率及六边形所占比例各参数差异无统计学意义(均P>0.05)。术后两组眼压不同时间点总体差异有统计学意义(F=6.252,P=0.036),其中术后3天、1周及2周两组间差异有统计学意义(P<0.05)。术后两组角膜内皮细胞密度、细胞多形性变异率以及六边形细胞所占比例差异均有统计学意义(F=13.878,5.842,13.090,P=0.041,0.019,0.021)。其中A组角膜内皮细胞密度于术后1周、2周、1个月、2个月及3个月降低,与B组差异有统计学意义(均P<0.05);A组多形性变异率于术后1周、2周、1个月、2个月及3个月均增加,与B组差异有统计学意义(均P<0.05);A组六边形细胞所占比例于术后2周、1个月、2个月及3个月均减少,与B组差异有统计学意义(均P<0.05)。B组角膜内皮细胞密度、细胞多形性变异率及六边形细胞所占比例各参数术前术后差异无统计学意义(均P>0.05)。结论PDR患者玻璃体切除硅油填充术后早期高眼压影响其角膜内皮细胞密度、多形性变异率及六边形细胞所占比例。Objective To observe the influence of early ocular hypertension after vitrectomy with silicone oil tamponade on corneal endothelial cells in patients with proliferative diabetic retinopathy(PDR).Methods This was a retrospective case-control study.A total of 60 eyes of 60 patients underwent vitrectomy with silicone oil tamponade from Jan.2018 to Mar.2019 in Affiliated Zhongshan Hospital of Dalian University were divided into two groups.In group A,30 eyes of 30 patients,early postoperative ocular hypertension occurred.In group B,30 eyes of 30 patients,postoperative intraocular pressure were normal.The two groups were compared and analyzed.Results There was no significant difference in intraocular pressure,corneal endothelial cell density,cell pleomorphic variation rate or the proportion of hexagonal cells before surgery between the two groups(all P>0.05).There was significant difference in intraocular pressure between the two groups at different time points(F=6.252,P=0.036),and there was significant difference between the two groups at 3 days,1 week and 2 weeks after operation(P<0.05).There were significant differences in the density of corneal endothelial cells,the rate of polymorphosis and the proportion of hexagonal cells between the two groups(F=13.878,5.842,13.090,P=0.041,0.019,0.021).The density of corneal endothelial cells in group A decreased at 1 week,2 weeks,1 month,2 months and 3 months postoperatively.The differences were statistically significant between the two groups(all P<0.05).The polymorphous variation rate in group A increased at 1 week,2 weeks,1 month,2 months and 3 months postoperatively.The differences were statistically significant between the two groups(all P<0.05).The proportion of hexagonal cells in group A decreased at 2 weeks,1 month,2 months and 3 months postoperatively.The differences were significant between the two groups(all P<0.05).There was no significant difference in the parameters of corneal endothelial cell density,cell pleomorphic variation rate,and the proportion of hexagona
