腹腔镜腹股沟淋巴结清扫术治疗阴茎癌的应用及疗效分析

摘　　要：目的 探讨腹腔镜腹股沟淋巴结清扫术治疗阴茎癌的应用措施及应用疗效.方法 选取我院2018年1月-2020年1月收治的阴茎癌患者40例,采用随机数字表法将患者分为两组,对照组采用传统开放性腹股沟淋巴结清扫术治疗,研究组采用腹腔镜腹股沟淋巴结清扫术治疗,比较两组患者手术出血量,手术时间、术后并发症、住院时间、手术创伤情况.结果 研究组术中出血量少于对照组术中出血量,差异有统计学意义(P<0.05);研究组手术时间、住院时间皆短于对照组,差异有统计学意义(P<0.05);两组患者术后并发症发生情况比较中,对照组并发症发生概率较研究组更高,差异有统计学意义(P<0.05);手术前两组患者血清Cor浓度、NE浓度指标比较,差异无统计学意义(P>0.05);手术后两组在Cor浓度与NE浓度上皆有提升,但研究组提升幅度小于对照组,差异有统计学意义(P<0.05).结论 腹腔镜腹股沟淋巴结清扫术用于阴茎癌患者中,能够减少术中出血,手术创伤较小,有利于患者术后恢复,且对患者造成的创伤影响较小,不产生较大的应激反应,术后并发症发生率低,临床疗效显著.Objective To explore the application measures of laparoscopic inguinal lymphadenectomy for the treatment of the patients with penis carcinoma and its efficacy. Methods A total of 40 patients with penis carcinoma admitted to our hospital from January 2018 to January 2020 were selected,and the patients were divided into two groups using the random number grouping.The control group was treated with traditional open inguinal lymphadenectomy and the study group was treated with laparoscopic inguinal lymphadenectomy.The bleeding volume, operation time,postoperative complications,hospitalization time,and surgical trauma were compared between the two groups. Results The intraoperative bleeding of the study group was less than that of the control group,the difference was statistically significant(P < 0.05);the operation time and hospital stay of the study group were shorter than those of the control group,the differences were statistically significant(P < 0.05);the comparison of the postoperative complication occurrence,the probability of complications in the control group was higher than that in the study group,the difference was statistically significant(P < 0.05);there was no statistical difference in serum Cor concentration and NE concentration indexes between the two groups before surgery(P > 0.05);after the operation,the concentration of Cor and NE in both groups increased,but the increase in the study group was smaller than that in the control group,and the difference was statistically significant(P < 0.05). Conclusion Laparoscopic inguinal lymphadenectomy for patients with penile carcinoma can reduce intraoperative bleeding,the surgical trauma is minor.It is beneficial to the patient’s postoperative recovery, has less stress reaction and lower incidence of postoperative complications,which is of great clinical efficacy.
